Definitive intensity-modulated radiation therapy for nasopharyngeal carcinoma: long-term outcome of a multicenter prospective study

Abstract: IMRT for NPC patients achieved excellent long-term locoregional control (LRC) and OS, with acceptable acute and late toxicities. After the treatment, xerostomia was improved over time. Distant metastasis remained the main cause of failure. More effective systemic therapy is demanding for reducing the risk of distant metastasis.

“…However, IMRT has its own risks, including marginal miss, which may lead to tumor recurrence 35,36 . Furthermore, even with the use of IMRT, a significant proportion of patients still develop long term xerostomia 37 . Even “dose sparing” approaches that preferentially minimize radiation to one of the two parotid glands may reduce salivary output of the spared gland by up to 50% 38 .…”

“…Radiation treatments to head and neck can damage salivary glands, causing a marked decrease in saliva production (21) Sjögren's syndrome is an autoimmune disease characterized by dryness of the mouth and eyes. Autoimmune diseases feature the abnormal production of extra antibodies in the blood that are directed against various tissues of the body (22).…”

“…However, no phase-III randomized trials have as yet compared IMRT versus 3D conformal RT. Several teams have published results, but retrospectively; the RTOG 0225 study is a non-randomized phase-II trial [13,14,16,[24][25][26][27][28][29][30][31] (cf. Table 2).…”
