Defining trapezial dysplasia – analysis of trapezial inclination in a normal population

Abstract: We assess the distribution of trapezial inclination in a young population in order to propose a threshold for trapezial dysplasia. One hundred peritrapezial views were reconstructed from wrist computed tomography scan datasets to measure trapezial inclination. Seventy peritrapezial views were constructed from 10 datasets to assess the influence of radiograph rotation. Mean trapezial inclination in our population was 8° (range 0.2–17.9). Fifteen degrees of radiographic pronation or supination did not alter trap… Show more

“…A dysplastic trapezium is defined as one with an increased slope of the distal articular surface with the potential to cause radial subluxation of the first metacarpal and instability of the TMC joint. If a patient has a dysplastic trapezium and symptomatic instability, an opening wedge trapezial osteotomy may be necessary to correct the slope of the distal articular surface and stabilize the forces around the TMC joint (Goubau et al., 2012; Van Royen et al., 2022). This procedure has shown to give good results at more than 10 years follow-up when combined with a metacarpal osteotomy (Thuysbaert et al., 2021).…”

“…Since these two joint surfaces are always situated in the same plane, the trapezial inclination is less variable than the trapezial tilt and is therefore a more reliable parameter to define trapezial dysplasia. The mean trapezial inclination is 8° and an inclination over 16° is considered to be dysplastic (Van Royen et al., 2022).…”
