Defining the optimum strategy for identifying adults and children with coeliac disease: systematic review and economic modelling

Abstract: Background Coeliac disease is an autoimmune disorder triggered by ingesting gluten. It affects approximately 1% of the UK population, but only one in three people is thought to have a diagnosis. Untreated coeliac disease may lead to malnutrition, anaemia, osteoporosis and lymphoma. Objectives The objectives were to define at-risk groups and determine the cost-effectiveness of active case-finding strategies in primary care. … Show more

“…Finally, as in the use of the RRRA renal registry for the NICE CKD model, data availability will play a key role. We will attempt to make best use of Clinical Practice Research Datalink data to which we will have access and which we used to success in a prior model for Coeliac disease screening [ 73 , 74 ].…”

“…In particular, one study included patients with an incidental finding of positive anti-tTG antibodies without clinical symptoms of CD, and as no small bowel biopsy was performed, it is possible that some of them might have had a form of asymptomatic CD ( 30 ). However, this laboratory finding in adulthood has widely variable sensitivity and specificity (35%–100% and 0%–100%, respectively) with consequent poor diagnostic significance ( 43 ) and the most recent meta-analysis on this topic, simulating the application of the obtained figures in a cohort of adult subjects with positive anti-tTG, which showed a percentage of false positives for CD of 87% ( 44 ).…”

“…The study benefited from a previous survey in which interviewees were purposefully sampled, including those with different stances on individual diagnostic confidence. 27 Sample size was appropriate and data saturation achieved.…”

“…Participants were purposefully sampled from 200 adults with a diagnosis of CD who had previously completed a survey on desired levels of diagnostic confidence before commencing a gluten-free diet and their views on the need for a biopsy. 27 Criteria used for sampling participants were age, sex, and level of diagnostic confidence (low, medium, or high). A total of 62 adults with CD, who had given permission to be contacted as part of the survey, were sampled and invited to participate via emails and a follow-up email sent at 2 weeks.…”
