Defining reliable disability outcomes in multiple sclerosis

Abstract: Prevention of irreversible disability is currently the most important goal of disease modifying therapy for multiple sclerosis. The disability outcomes used in most clinical trials rely on progression of Expanded Disability Status Scale score confirmed over 3 or 6 months. However, sensitivity and stability of this metric has not been extensively evaluated. Using the global MSBase cohort study, we evaluated 48 criteria of disability progression, testing three definitions of baseline disability, two definitions … Show more

“…Disability accumulation was defined as an on-treatment increase in EDSS by 1 step (1.5 steps if baseline EDSS was 0 and 0.5 steps if baseline EDSS was >5.5) confirmed by subsequent EDSS scores over ≥6 months (irrespective of treatment status at confirmation). Disability improvement was defined as a decrease in EDSS by 1 step (1.5 steps if baseline EDSS was 1.5 and 0.5 steps if baseline EDSS was >6) confirmed over ≥6 months 22. Treatment discontinuation events and their main reasons were recorded; these reasons did not use unified definitions the reasons for treatment discontinuation were reported as per treating neurologists.…”

“…The choice of confirmation time for baseline and end point EDSS score was based on greater stability of disability progression events at higher EDSS scores 16. Patients not attaining this outcome were censored at their last recorded EDSS score.…”

“…Of note, recent evaluation of disability outcomes from the MSBase cohort, based on 3-6 months of CDP on EDSS scores, suggests that the accumulation of permanent disability may be overestimated by up to 30%, which may lead to spurious estimates from short-term clinical trials [Kalincik et al 2015a]. …”
