Background: Diabetes is a chronic disease with severe late complications. It is known to impact the quality of life and cause disability, which may affect an individual’s capacity to manage and maintain longer-term health and well-being. Objectives: To examine the prevalence of self-report diabetes, and association between diabetes and each of health-related quality of life and disability amongst South Africa’s older adults. To study both the direct relationship between diabetes and these two measures, as well as moderation effects, i.e. whether associations between other factors and these measures of well-being differed between individuals with diabetes and those without. Methods: Secondary analyses of data on participants aged 50 years and older from the Study on global AGEing and adult health (SAGE) in South Africa Wave 1 (2007–2008) were conducted. Prevalence of self-reported diabetes was assessed. Multivariable regressions describe the relationships between each of quality of life (WHOQoL) and disability (WHODAS), and diabetes, while controlling for selected socio-demographic characteristics, health risk behaviours and co-morbid conditions. In the regression models, we also investigated whether diabetes moderates the relationships between these additional factors and WHOQoL/WHODAS. Results: Self-reported diabetes prevalence was 9.2% (95% CI: 7.8,10.9) and increased with age. Having diabetes was associated with poorer WHOQoL scores (additive effect: −4.2; 95% CI: −9.2,0.9; p-value <0.001) and greater disability (multiplicative effect: 2.1; 95% CI: 1.5,2.9; p-value <0.001). Lower quality of life and greater disability were both related to not being in a relationship, lower education, less wealth, lower physical activity and a larger number of chronic conditions. Conclusions: Diabetes is associated with lower quality of life and greater disability amongst older South Africans. Attention needs to be given to enhancing the capacity of health systems to meet the changing needs of ageing populations with diabetes in SA as well as facilitating social support networks in communities.

RESUMOEste estudo teve como objetivo analisar a qualidade de vida de acadêmicos ingressantes em cursos de Educação Física e compará-la entre os sexos, cursos, status do peso, imagem corporal e níveis de atividade física dos participantes. Participaram 198 acadêmicos (Bacharelado= 104 e Licenciatura= 94), com média de idade de 20,6 (4,6) anos. Os participantes responderam a um questionário sobre sexo, idade, massa corporal e estatura (para cálculo do IMC e classificação do status do peso), imagem corporal (escala de silhuetas), nível de atividade física (IPAQ curto) e qualidade de vida (WHOQOL-Bref). A média da qualidade de vida geral foi de 74,9 pontos. Verificou-se melhor qualidade de vida nos domínios físico e social e pior no ambiental. Observaram-se diferenças na qualidade de vida geral e nos domínios físico, psicológico e social entre as categorias da imagem corporal. Houve diferença também na qualidade de vida geral e nos domínios físico e ambiental entre os níveis de atividade física. Conclui-se que os acadêmicos apresentaram boa qualidade de vida. Comparados aos seus pares, os insatisfeitos com sua imagem corporal tinham qualidade de vida inferior e os ativos fisicamente, superior. Não houve diferenças na qualidade de vida entre os sexos, cursos ou status do peso. Palavras-chave: Estudantes. Educação física. Qualidade de vida. Imagem corporal. Atividade física. ABSTRACTThis study aimed to analyze the quality of life in freshmen college students of Physical Education courses and compare it between sex, courses, weight status, body image and physical activity level of the participants. Participants were 198 college students (Bachelor= 104 and Licentiate= 94), with a mean age of 20.6 (4.6) years. Participants answered a questionnaire about their sex, age, body mass and height (for BMI calculation and weight status classification), body image (figure rating scale), physical activity level (IPAQ short form) and quality of life (WHOQOL-Bref). Mean score of quality of life was 74.9 points. Better quality of life was founded in physical and social domains and worst quality of life in environmental domain. Differences were observed in general quality of life, and physical, psychological and social domains between body image categories. There was also difference in general quality of life and physical and environmental domains between physical activity levels. Concluding, college students had good quality of life. Compared to peers, the ones dissatisfied with their body image had lower quality of life and the physically actives, higher. Long-term quality of life following anterior cruciate ligament injury and reconstruction

The anterior cruciate ligament (ACL) serves to stabilise the knee complex and prevent intraarticular knee structures from sustaining damage during multiaxial, high impact movement. An ACL rupture most commonly occurs in adolescents and young adults taking part in competitive sports and causes substantial disruption to the knee joint. Anterior cruciate ligament reconstruction (ACLR) is the most common treatment offered to ACL-ruptured individuals worldwide. Individuals undergoing this surgery often have high expectations that rarely match surgical outcomes. Return to sport rates are low, re-injury fears are widespread and many individuals develop knee osteoarthritis within ten years of ACLR. This mismatch between expectations and actual outcomes has great potential to impact longer-term quality of life (QOL). The primary aims of this thesis were to: (i) generate a detailed picture of longer-term QOL following ACLR, and (ii) identify factors related to longer-term QOL in people with knee pain, symptoms or functional limitations following ACLR.The first study in this thesis is a systematic review that revealed impaired knee-related QOL at 5 to 20 years after ACLR. However, it became evident that research into factors impacting upon QOL in this population was scarce. The systematic review found that having a subsequent knee injury, an additional knee surgery after ACLR, and developing severe radiographic osteoarthritis were associated with worse QOL outcomes. The second study in this thesis is a systematic review that investigated QOL more than five years after ACL rupture in individuals who remained ACL-deficient (did not undergo ACLR) and compared QOL between ACLdeficient and ACL-reconstructed groups. This systematic review found that ACL-deficient individuals reported comparable QOL outcomes to those who underwent ACLR 5 to 25 years previously. All studies that reported QOL after ACLR included asymptomatic participants without knee difficulties, which may underestimate the degree of QOL impairment experienced by people with ongoing knee difficulties.The third study in this thesis used a cross-sectional design to describe QOL outcomes in 162 individuals with knee pain, symptoms or functional limitations 5 to 20 years following ACLR.It showed that individuals who did not return to sport after ACLR reported poorer knee-related and health-related QOL, compared to people who returned to pre-injury levels of sport. Higher body mass index (BMI) and subsequent surgery were also related to worse QOL after ACLR.This was the first study to show that return to sport was associated with QOL impairment more than five years after ACLR. However, quantitative assessment of QOL may overlook important considerations with regard to an individual's life priorities, expectations, values and concerns.To provide a detailed understanding of QOL following ACLR, qualitative interviews were performed with 17 individuals with knee difficulties 5 to 20 years following ACLR.
