Defining hip pain trajectories in early symptomatic hip osteoarthritis – 5 year results from a nationwide prospective cohort study (CHECK)

Abstract: We defined four distinct pain trajectories over 5 years follow-up in individuals with early symptomatic hip OA, suggesting there are differences in symptomatic progression of hip OA. Baseline radiographic severity was not associated with the pain trajectories. Future research should be aimed at measuring symptomatic progression of hip OA with even more frequent symptom assessment.

“…In 9 assessments of a domain (3%) in 6 different studies, a third reviewer made the final judgment. In total, 15 studies scored a low risk of bias in all domains [15, 16, 21, 29, 30, 32, 34, 37, 41, 44, 47, 49, 53, 57, 63] (Table 2).…”

“…There are a few potentially important risk factors for HOA for which data were not available in our study including hip injury, physical activity, and physical examination of the hips (RS cohort). The studies among CHECK participants 23,24 have shown that painful internal rotation of the hip is a good predictor of 6 years THR. Moreover, our validation cohorts lacked some of the model variables such as family history of OA (RS-II and CHECK), and hand OA (CHECK) which could result in underestimation of the predicted probabilities.…”

“…There are 2 main approaches to identifying latent class trajectories within a heterogeneous population: group based trajectory model (GBTM) or growth mixture model (GMM) . These 2 approaches have been used to study juvenile arthritis, rheumatoid arthritis, and osteoarthritis . We chose GMM, as the concept of this model was consistent with our clinical observation that there is clustering of different disease courses.…”
