Suboptimal teamwork in the operating room (OR) is a contributing factor in a significant proportion of preventable complications for surgical patients. Specifying behaviour is fundamental to closing evidence-practice gaps in healthcare. Current teamwork interventions, however, have yet to be synthesized in this way. This scoping review aimed to identify actionable strategies for use during surgery by mapping the existing literature according to the Action, Actor, Context, Target, Time (AACTT) framework. The databases MEDLINE (Medical Literature Analysis and Retrieval System Online), Embase, Cumulated Index to Nursing and Allied Health Literature (CINAHL), Education Resources Information Center (ERIC), Cochrane, Scopus, and PsycINFO were searched from inception to April 5, 2022. Screening and data extraction were conducted in duplicate by pairs of independent reviewers. The search identified 9,289 references after the removal of duplicates. Across 249 studies deemed eligible for inclusion, eight types of teamwork interventions could be mapped according to the AACTT framework: bundle/checklists, protocols, audit and feedback, clinical practice guidelines, environmental change, cognitive aid, education, and other), yet many were ambiguous regarding the actors and actions involved. The 101 included protocol interventions appeared to be among the most actionable for the OR based on the clear specification of ACCTT elements, and their effectiveness should be evaluated and compared in future work.

SummaryPeri‐operative medication safety is complex. Avoidance of medication errors is both system‐ and practitioner‐based, and many departments within the hospital contribute to safe and effective systems. For the individual anaesthetist, drawing up, labelling and then the correct administration of medications are key components in a patient's peri‐operative journey. These guidelines aim to provide pragmatic safety steps for the practitioner and other individuals within the operative environment, as well as short‐ to long‐term goals for development of a collaborative approach to reducing errors. The aim is that they will be used as a basis for instilling good practice.

The project's objective is to make HSE more accessible and tangible in acquiring competencies and transferring HSE training content to practice by showing its relevance concerning a novel operation protocol. Although individually CRM has a high potential to improve the safety, health, and environmental (HSE) culture of individuals and reduce errors in various areas, its implementation in the construction industry and an integrated manner has not been investigated. The authors investigated the effect of the implementation of HSE-CRM in a project for a housing complex in Karaj, with 230 staff. Despite the assessment that said this training is slightly more difficult, staff reacted more positively to this training than to standard training. Across all responses, there was statistically considerable growth in scores on the HSE culture survey items. Also, this process improved teamwork, performance, efficiency, and staff morale. Most participants said this training format was seen as livelier, more interesting, and more practically relevant. After one year, participants claimed this approach protected the HSE Chief and other managers from distractions and wasted time at the beginning of each shift. The result of the project shows that this technique can be used in the environment of construction.
