Defining and evaluating a novel outcome measure representing end-stage knee osteoarthritis: data from the Osteoarthritis Initiative

Abstract: Objective We described a definition of end-stage knee osteoarthritis (esKOA) and evaluated its association with health outcomes and osteoarthritis risk factors. Method We included Osteoarthritis Initiative participants with or at-risk for knee osteoarthritis who had complete baseline data. We defined esKOA by adapting a validated appropriateness algorithm for total knee replacement based on data from baseline and the first 4 follow-up visits. We performed person-based analyses, including both knees from all … Show more

“…End-stage KOA has been viewed as a knee with considerable pain and functional limitations, accompanied by structural damage and/or other complications, such as flexion contractures and joint laxity, that prohibit the normal use of a joint (Driban et al, 2016). Generally, end-stage KOA is characterized as KL grade 4 on radiography (Guermazi et al, 2015).…”

“…The data we use in this study were collected according to the OAI protocol. As Driban et al mention, the OAI study was not designed to model eskOA 7 . In particular, the limited knee mobility condition in the eskOA definition uses variables that the OAI does not measure each year.…”

“…To determine which patients have both advanced structural and symptomatic OA, we use an end-stage knee OA (eskOA) definition from Driban et al 7 that incorporates symptomatic, radiographic, and knee mobility criteria. Our model presents a risk score for this outcome.…”

“…However, for all the mentioned treatment options, the guidelines do not provide clear guidance about when surgery is most beneficial for the individual patient. Consequently, under or over utilisation of healthcare resources may occur [2,[9][10][11]. Research suggests that up to 75% of patients could have postponed surgery and managed with physical training instead [12].…”
