Deep venous thrombosis after Achilles tendon rupture is associated with poor patient-reported outcome

Abstract: The aim of this study was to investigate whether patient subjective and functional outcomes after Achilles tendon rupture (ATR) are related to deep venous thrombosis (DVT) during leg immobilization. Methods A cohort study with prospectively collected randomized data was conducted between 2010 and 2017. Two-hundred and fifty-one Patients with an Achilles tendon rupture (mean age = 41 ± 8), treated with uniform surgical techniques, were retrospectively analyzed. DVT incidence at 2 and 6 weeks was assessed using … Show more

“…PTS is a syndrome characterized by varying grades of pain, heaviness, swelling, and chronic ulceration of the leg 240 . DVT impairs long-term health-related quality of life and may negatively affect treatment results after ATRs 241,242 . The duration of immobilization in a below-the-knee cast and/or an orthosis/walking boot following an ATR is usually 6-8 weeks.…”

“…DVTs after Achilles tendon rupture consists mainly of distal (below the knee) DVTs and are associated with a low risk of developing postthrombotic syndrome 338 . However, sustaining a DVT during leg immobilization significantly impairs patient-reported outcome 242 . The incidence of DVTs might be higher in patients older than 40 years, presumably because of a higher incidence of congestive heart failure, history of DVT or PE, and obesity 107,244 .…”

“…37 Our study demonstrated 10 incidences (8.50%) of DVT in the MIS group and 6 incidences (9.00%) of DVT in the open group (P > .99), which falls within the reported range in the literature from below 1% up to 50%. 2,3,27,29,30,33,37 Procedure duration was the only statistically significant difference found between the MIS and open groups, but only differed by 3.9 minutes on average, which may not factor in the decision to use either technique.…”

“…There is evidence that delay from injury to surgery and short duration of surgery are associated with inferior patient outcomes after ATR in addition to older age, obesity, and presence of deep venous thrombosis (DVT). 2,12,17,20 22,26 Ideally, surgical training should not affect patient outcomes significantly. In orthopaedic surgery, however, there is some evidence that the level of surgeon experience may influence long-term outcomes.…”
