Deep Vein Thrombosis and Changes in Coagulation and Fibrinolysis after Gynaecological Operations in Chinese: the Effect of Oral Contraceptives and Malignant Disease

Abstract: Of 154 Chinese patients who underwent gynaecological operations, four showed a positive 125I fibrinogen leg scan for venous thrombosis, and overall incidence of 2 . 6%. In those who were on oral contraceptives and had major pelvic surgery for benign conditions, the incidence was 10 . 5%; in those who had Wertheim hysterectomy for carcinoma of cervix, it was 6 . 7%. This confirms the rarity of post-operative thromboembolism in the Chinese. Fragment E showed a biphasic rise after major operation due to tissue in… Show more

“…Crossed immunoelectrophoresis of AT III in plasma and serum showed changes following surgery. A decrease of these inhibitors fol lowing major surgical procedures has been previously described by several authors [25][26][27], Our results are consistent with those described by Gitel et al [28] and Gray et al [29], both of whom found no alterations in AT III levels after total hip replacement. With respect to the alterations found in crossed electrophoresis of AT III it has been suggested by several authors that they could be due to AT III complexes with thrombin and other serine proteases [30][31][32].…”

“…A sig nificant postoperative increase in the concen trations of fibrinogen, factor VI1I:C and fac tor VIIIR:Ag was present, while factor Xa levels and electrophoretic mobility of factor VIII remained unchanged. Similar results were reported by Houghton et al [24] after total hip replacement, and by Tso et al [25] following gynecological procedures. No alter ations were found in the mean concentration of AT III and antifactor Xa in our series.…”

“…According to some ethnic-comparison studies, the incidence among Asian-Americans and Asian-Pacific Islanders appears to be 2.5-fold to 4-fold lower than that among Caucasians [8][9][10]. Symptomatic VTE after surgery has been found to be rare in studies from East Asia [11,12]. Thus, we hypothesized that the incidence and recurrence rate of VTE may be lower in the Taiwanese population than in Caucasians.…”

“…Venous thromboembolism (VTE) has been considered rare among Chinese [1,2], but recent studies have showed an increasing trend [3,4]. The reason for its increased trend in the Chinese population remains unclear.…”
