Background: Small airways disease (SAD) is a key risk in developing obstructive lung diseases (OLD). Handgrip strength (HGS) is found to be associated with pulmonary function in populations with lung conditions. Hispanics remain the main workforce in farming industry, but their prevalence of lung conditions remain understudied. Likewise, HGS also remains understudied in Hispanic and farmworker populations. Our study investigated the prevalence of SAD and OLD as well as their associations with HGS among Hispanic farmworkers. Methods: A cross-sectional study analyzed 113 Hispanic farmworkers (54% female) who were screened using pulmonary function tests during annual health fairs in rural Southeastern US from 2013 to 2017. Smoking status was self-reported. SAD was defined as forced expiratory flow at 25–75% predicted of vital capacity (FEF25 − 75% predicted) ≤ 60% per literature and OLD defined as forced expiratory volume in 1 second/ forced vital capacity (FEV1/FVC) ratio < 70% per Global Initiative for Chronic Obstructive Lung Disease criteria. Seated isometric absolute (the sum of both hands) and relative (absolute handgrip strength divided by body mass index) handgrip strengths were collected. Results: 26.5% of subjects had SAD and 15.9% had OLD. 50% of subjects with SAD had OLD while 83% of subjects with OLD had SAD. 13% of overall population smoked. Lower absolute and relative HGS groups had higher prevalence of SAD and OLD. Multivariate linear regression showed that lower absolute and relative HGS were associated with worsened small airway function. Age and FEF25 − 75% predicted were associated with FEV1/FVC. Smoking, body mass index, blood pressures, hemoglobin A1C and lipids were not predictors in either model. Conclusions: This is one of the first studies reporting prevalence of pulmonary function in Hispanic farmworkers. Although this population was relatively young and healthy, there was high prevalence of SAD and OLD, which was higher than the overall prevalence in Hispanic population. There were more females subjects with SAD. Most of the subjects with OLD had SAD but not vice versa. Lower HGS levels were associated with worsened pulmonary function, and HGS was a significant predictor of FEF25 − 75% predicted, a potential marker for small airway physiology.

Objective: The present study investigated the possible changes in respiratory function and mechanics in paralympic cyclists in comparison with cyclists. Design: Lung function (spirometry) and mechanics (impulse oscillometry) pre- and post-bronchodilator, the levels of FeNO and the peripheral and respiratory muscle strength were evaluated in paralympic cyclists [spinal injury from C5 (incomplete injury) until T11 (complete injury); (n=8)] and cyclists (n=12). Results: Paralympic cyclists presented reduced FVC, FEV1, PEF, VC In and MEF75%. Of note, only athletes presented response to bronchodilator, as observed for FEV and for FVC/FEV1. Respiratory impedance (Z5Hz), total respiratory system resistance (R5Hz) was impaired in paralympic cyclists. No differences were found hand grip and respiratory muscle strength and for FeNO comparing paralympic cyclists with cyclists. Conclusion: We conclude that para-cyclists present impaired lung function and mechanics and that such impairments are not correlated with reduction in the respiratory or peripheral muscle strength or with pulmonary inflammation.

RESUMEN: Los pacientes con COVID-19 subsidiarios de ventilación mecánica (VM), evolucionan con consecuencias funcionales en la musculatura ventilatoria y apendicular que no necesariamente se abordan de manera diferenciada. El objetivo de esta investigación fue evaluar el impacto de un programa de recuperación funcional en estos pacientes y determinar si las intervenciones afectan de manera diferenciada a las funciones ventilatorias y musculatura apendicular, utilizando pruebas de bajo costo. Se evaluaron 47 pacientes con COVID-19 que estuvieron en VM. Posterior a una espirometría basal se les realizó; presión inspiratoria máxima (PIMáx), fuerza de prensión palmar (FPP), prueba de pararse y sentarse (PPS) y Prueba de caminata en 6 minutos (PC6m), antes y después del plan de intervención. Este programa incluyó ejercicios aeróbicos y de fuerza supervisados por dos sesiones semanales de 60 minutos durante 3 meses. Después del programa, se observaron mejoras significativas en la capacidad vital forzada (CVF), el volumen espiratorio en el primer segundo (VEF1) y la PIMáx. Se encontraron relaciones significativas entre estas mediciones y la distancia recorrida de la PC6m, la FPP y la PPS. En conclusión, el programa de recuperación funcional en pacientes con COVID-19 que requirieron VM, beneficia tanto la función ventilatoria como la fuerza muscular apendicular. Las pruebas de fuerza muscular apendicular pueden ser útiles para evaluar la recuperación ya que pueden entregar información diferenciada de sus rendimientos. Por último, se necesita más investigación para comprender mejor la respuesta de estos pacientes a la rehabilitación.
