Resumen: Introducción. El quiste radicular es el quiste más común de los maxilares. Su tamaño puede variar desde una lesión que abarca el proceso alveolar hasta uno extenso que oblitera el espacio antral maxilar o causa una fractura patológica mandibular. Reporte de casos. Se reportan tres casos con fractura patológica mandibular asociados a quiste radicular ocurridos después de trauma facial. El diagnóstico fue sugerido por un estudio clínico, radiografía panorámica y confirmado por el estudio histopatológico de la pieza operatoria. El tratamiento consistió en enucleación quística seguido por inmovilización de fragmentos con osteosíntesis o bloqueo intermaxilar. El resultado clínico y radiológico a corto plazo fue favorable. Discusión. Se discuten aspectos propios de la patología y su terapia. Conclusión. El éxito del tratamiento depende de una adecuada terapia en que sus principios más importantes son la remoción de la lesión mediante enucleación y una fijación estable.

Objective: There are some things that should be considered in treating growing patient with odontogenic cyst. The marsupialization is considered to have more benefits compared to enucleation for growing patient in order to overcome treatment morbidity such as damage to vital structures, heavy loss of bone, jaw growth disturbances, and pathological fracture. This paper reports the success of two cases of marsupialization of odontogenic cysts in order to reduce the treatment morbidity and to discuss about enucleation for young patient.Methods:Two female 21 and 22 years old with radicular cysts on tooth 36. Marsupialization was done before completing cyst enucleation.Results: The shrinkage of the cyst was observed periodically. The position of the wall of the cyst was superficially positioned and the enucleation was done. The treatment result was satisfying with low morbidity.Conclusion:Marsupialization for odontogenic cyst in growing patient has more advantages than enucleation to reduce treatment morbidity.

Background: Radical cysts are the most common inflammatory odontogenic cystic lesions that can be treated by Marsupialization, especially if the cyst is large and has vital structures. There are a few things to consider when treating a growing patient with an odontogenic cyst. Marsupialization has more benefits for the growing patient than enucleation to overcome treatment morbidity, such as damage to vital structures, bone loss, jaw growth disorders and pathological fractures. Purpose: This article reports on the success of marsupialization of odontogenic cysts "that have been associated with very decaying deciduous molars" to reduce treatment morbidity and to discuss enucleation in young patients. Materials and Methods: A healthy 9 patients with average age of 8 years old with unilateral radicular mandibular cysts. Marsupialization was done before the end of cyst enucleation incorporating a multipurpose space maintainer. Results: Periodic contraction of the cysts was observed in all patient cystic site. The position of the cyst wall was placed superficially and an enucleation was performed. The treatment result was satisfactory with low morbidity. Conclusion: Marsupialization with a versatile space maintainer for the odontogenic periapical cyst in growing patients has more advantages than enucleation in reducing treatment morbidity.
