“…For example, in Japan, an analysis of data drawn from the Institute of Rheu matology, Rheumatoid Arthritis (IORRA) study, between 2000 and 2007, reveals that synovectomy procedures have diminished fi vefold following the introduction of methotrexate in 1999 [ 47 ]. Moreover, patients undergoing surgery present with milder disease compared with patients in previous years [ 47 -54 ].…”

“…Based on a constant mean number of RA patients seen each reporting period, the number of synovectomies peaked in 2000, and has been gradually decreasing since MTX approval [17]. Although some studies have reported that tumor necrosis factor-a (TNFa) inhibitors have retarded the progression of joint damage as assessed radiographically, and that these treatments may have caused a decline in orthopedic surgery, the results from IORRA indicate that the decline in synovectomy surgery has been mainly influenced by MTX treatment [17]. Regardless, synovectomy is still required if arthritis persists in a joint despite aggressive medical treatment.…”

“…The rate of synovectomy surgery was first examined in the IORRA cohort study [17]. Synovectomy was usually performed on joints that were persistently painful for at least 6 months, did not respond to medical management, and in which no radiographic progression was observed [18].…”
