Death due to external compression of the trachea in a patient with multinodular hemorrhagic goiter

Abstract: In this paper we describe the case of an 81-year-old Caucasian female (142 cm tall, weighing 45 kg) who suffered from a multinodular goiter for approximately 40 years. Following the onset of a clinical condition characterized by acute respiratory failure, she was transported to the emergency room by ambulance, where she died within a few hours after admission. A recent cardiac examination showed the absence of risk factors for cardiovascular disease, sinus tachycardia with a heart rate of 131 bpm, negative obj… Show more

“…Two possible mechanisms responsible for bleeding have been described: anomalous vascularization of a multinodular goiter, which increases the fragility of the vessels and hence the probability of their rupture, and the creation of arteriovenous shunts in the setting of thyroid parenchyma with disrupted architecture. 19,20 In the case reported here, disruption of the parenchymal structure was observed. Indeed, histological examinations revealed a colloid nodular goiter with severe fibrosclerotic sequelae secondary to previous thyroiditis.…”

“…Such a clinical presenta-tion is rare, occurring in only up to 0.6% of cases (7). Therefore, this initial manifestation of thyroid dysfunction in the postoperative course may lead to a false diagnosis and potentially a lethal event (8). In such situations, rapid diagnosis and early surgery are crucial to prevent respiratory failure and a potentially lethal outcome.…”
