De Garengeot’s Hernia: Report of a Rare Surgical Emergency and Review of the Literature

Abstract: This is a report of a case who was admitted and operated on for a strangulated femoral hernia. The hernia sac contained a gangrenous appendix, which was excised and the hernia was repaired with sutures without complication. De Garengeot's hernia, although very rare, should be included in the differential diagnosis of cases with strangulated hernia and should receive the optimal treatment.

“…We recommend avoidance of mesh-based repair techniques in the setting of appendicitis, specifically perforated cases. Advocates of pure laparoscopic approaches support potential advantage in a shorter hospital stay and speedier recovery [ 9 ]. The most common complications are wound infections [ 5 ].…”

“…Appendicectomy via delivery of the entire appendix through the femoral canal or inguinal incision has been widely used with a primary hernia repair [ 2 , 3 , [12] , [13] , [14] , [15] , [16] , [17] ]. If the appendix is clearly visualised and not inflamed, Sharma suggests repair of the hernia following reduction of the normal appendix is acceptable [ 18 ].…”

“…1 The abnormal pelvic location of the appendix may allow the migration of the appendix into the inguinal hernia or, as in our case, into a femoral hernia. 2 We are presenting a case of De Garengeot hernia which was treated with an open repair with mesh.…”

“…1 Preoperative diagnosis of De Garengeot hernia is challenging and only 2 reports of a preoperative CT diagnosis are available in the literature. 2 When identified, repair is performed urgently using a variety of approaches, including laparoscopic appendectomy and primary repair of the femoral hernia or open techniques with or without mesh placement. 3 Although there is no formal consensus on the optimal approach to repair these hernias, there is generally an agreement against the use of synthetic mesh in cases with well documented inflammation.…”
