Day of surgery cancellation rate after preoperative telephone nurse screening or comprehensive optimization visit

Abstract: BackgroundStructured preoperative assessment has been reported to improve operating room efficiency as measured by metrics such as day of surgery cancellations (DOSCs). However, not all patients require comprehensive assessment; routine full assessments can result in unnecessary duplication of tests and investigations. Selective nurse screening under the supervision of anesthesiology may provide adequate information gathering in lower risk patients. This study is undertaken to assess if DOSC rates vary with di… Show more

“…Patients’ vital signs (blood pressure, heart rate, body temperature, and oxygen saturation) should be measured accurately at their ward by the ward nurse and should be checked by the anesthesiologist. A structured preoperative assessment has been reported to improve operating room efficiency and reduce surgery cancellations [10]. A standardized preoperative evaluation checklist can also reduce the number of cancellations [11].…”

“…[10][11][12][13] Even with advancements in hospital quality measures directed toward decreasing patient cancellation rates, these "avoidable" cancellations continue to occur. 8,14,15 Despite the increased interest in surgical cancellations, knowledge regarding the factors that influence patient cancellations is incomplete. Currently, there are limited data on determining which patients are at high risk of elective orthopaedic surgery cancellation.…”

“…Its continued popularity with some anaesthetists and notably non-anaesthetist emergency physicians and intensivists is defended by assertions that single doses do not impact morbidity/mortality, although this remains controversial. Novel etomidate derivatives have dealt with the adrenocortical depression issue, through structural changes to form a pyrrole analogue, carboetomidate 4 with…”

“…Various terms are used to describe the reason for these preoperative medical evaluations, including risk assessment, 1 risk stratification, 2 medical clearance, 3 and optimization. 4 But it is not only the terminology that varies, the practice of preoperative medical evaluations varies widely between providers and hospitals. 5 6 This imprecision in terminology and variation in practice may reflect conceptual confusion regarding surgical risk, which may contribute to misuse or overuse of preoperative services.…”
