Background: Previous studies investigating factors associated with healthcare utilization by older Ghanaians lack distinction between public and private health services. The present study examined factors associated with public and private healthcare service use, and the resulting perceived health system responsiveness. Objectives: To identify factors associated with public and private healthcare utilization among older adults aged 50 and older in Ghana; and to compare perceived differences in health system responsiveness between the private and public sectors. Methods: Cross-sectional data was analyzed from the World Health Organization Study on global AGEing and adult health (SAGE) Wave 1 in Ghana. Using Andersen’s conceptual framework, public and private outpatient care utilization was examined using multinomial logistic regression to estimate and identify predictor variables associated with the type of outpatient healthcare facility accessed. Health system responsiveness was compared using chi-square tests. Results: Of 2517 respondents who used outpatient care in the 12 months preceding interview, 51.7% of respondents used a public facility, 17.8% a private facility, and 30.5% used other facilities. Older age group, higher education and higher wealth were associated with the use of private outpatient healthcare services. Using public outpatient care facilities was associated with having health insurance. Respondents with two or more chronic conditions were more likely to use public and private outpatient care than other facilities. Perceived health system responsiveness was better in private for-profit than in public and private not-for-profit healthcare facilities. Conclusions: This study suggested that higher wealth and multimorbidity were significant predictors of public and private outpatient healthcare utilization; however, health insurance was a predictor only for the use of public facilities. Future mixed-method studies could further elucidate factors influencing the choice of public and private outpatient healthcare use.

Background: Diabetes is a chronic disease with severe late complications. It is known to impact the quality of life and cause disability, which may affect an individual’s capacity to manage and maintain longer-term health and well-being. Objectives: To examine the prevalence of self-report diabetes, and association between diabetes and each of health-related quality of life and disability amongst South Africa’s older adults. To study both the direct relationship between diabetes and these two measures, as well as moderation effects, i.e. whether associations between other factors and these measures of well-being differed between individuals with diabetes and those without. Methods: Secondary analyses of data on participants aged 50 years and older from the Study on global AGEing and adult health (SAGE) in South Africa Wave 1 (2007–2008) were conducted. Prevalence of self-reported diabetes was assessed. Multivariable regressions describe the relationships between each of quality of life (WHOQoL) and disability (WHODAS), and diabetes, while controlling for selected socio-demographic characteristics, health risk behaviours and co-morbid conditions. In the regression models, we also investigated whether diabetes moderates the relationships between these additional factors and WHOQoL/WHODAS. Results: Self-reported diabetes prevalence was 9.2% (95% CI: 7.8,10.9) and increased with age. Having diabetes was associated with poorer WHOQoL scores (additive effect: −4.2; 95% CI: −9.2,0.9; p-value <0.001) and greater disability (multiplicative effect: 2.1; 95% CI: 1.5,2.9; p-value <0.001). Lower quality of life and greater disability were both related to not being in a relationship, lower education, less wealth, lower physical activity and a larger number of chronic conditions. Conclusions: Diabetes is associated with lower quality of life and greater disability amongst older South Africans. Attention needs to be given to enhancing the capacity of health systems to meet the changing needs of ageing populations with diabetes in SA as well as facilitating social support networks in communities.

Diseases of the Rich? The Social Patterning of Hypertension in Six Low- and Middle-Income Countries

Introduction.Hypertension and non-communicable diseases (NCDs) are not as yet priority issues for international development. This is surprising, since NCDs already account for a higher share of mortality and illness in low and middle income countries (LMICs) than infectious diseases do, and are set to rapidly increase (WHO, 2010). Referring to the broader policy literature, the papers identifies reasons for this general neglect. These include a general perception that, in contrast to high income countries, in LMICs NCDs disproportionately affect relatively wealthy groups. Drawing on survey data for six countries, we examine the validity of this perception with particular reference to hypertension prevalence, awareness and effective control. In conclusion, we considers the wider applications of its findings for agenda-setting for NCDs in global health. NCDs, hypertension and international development priorities.NCDs include a range of health conditions, such as cardio-vascular diseases, cancers and diabetes. In 2008 they accounted for 63 per cent of all deaths globally, and 80 per cent of these deaths occurred in LMICs (WHO, 2010). In all world regions except Africa the number of deaths caused by NCDs in 2008 already exceeded those caused by infectious, maternal, perinatal and nutritional diseases combined. It is projected that this will be the case for Africa by 2030 (WHO, 2010. NCDs also account for around half the global burden of illness and disability (Beaglehole et al, 2011).Hypertension is by some distance the leading risk factor for NCDs. WHO estimates that in 2008 hypertension was responsible for about 13 per cent of all global deaths (WHO, 2010).
