Data-driven method to infer the seizure propagation patterns in an epileptic brain from intracranial electroencephalography

Abstract: Surgical interventions in epileptic patients aimed at the removal of the epileptogenic zone have success rates at only 60-70%. This failure can be partly attributed to the insufficient spatial sampling by the implanted intracranial electrodes during the clinical evaluation, leading to an incomplete picture of spatio-temporal seizure organization in the regions that are not directly observed. Utilizing the partial observations of the seizure spreading through the brain network, complemented by the assumption th… Show more

“…In fact, the SI model as it was used here corresponds to the highly-super critical case in which seizures always spread. Thus, in order to avoid more assumptions that would hinder the interpretation of the outcomes of such a model, the SIR parameters would need to be tuned with clinical data, using for instance high-resolution spatio-temporal seizure-propagation patterns [36,44,89].…”

“…However, in order to do this realistically and in a systematic manner, more detailed spatio-temporal patterns of seizure propagation are needed for each patient. These could be obtained from the SEEG recordings directly [36,44,89].…”

“…Even though these coupling definitions are different, they have been used for similar purposes in the literature. For example, in the epilepsy literature, the additive coupling has been used in studies to distinguish functional networks from healthy people and people with epilepsy [12], to simulate epilepsy surgery [6,13,14], and to model seizure propagation [15]. On the other hand, the diffusive coupling has also been used in November 25, 2021 2/33 studies to differentiate controls from people with epilepsy [11], to investigate epilepsy surgery [10,16,17], and to understand patterns of seizure emergence [18].…”

“…Inferences can be further improved by integrating prior knowledge about the spatial distribution of the EZN, such as that obtained from the spatial extent of lesions in MRI data or seizure semiology, to reduce uncertainty and to bias the non-identifiable parameter estimation. After the first proof-of-concept of the VEP 11 , we studied and tested different modeling methods [12][13][14][15] , model inversion methods [16][17][18][19] , and surgery strategies 20,21 . Here, we present a complete and extensively tested VEP workflow that can be used in clinical practice.…”
