Abstract:The organizers of the first EFLM Strategic Conference "Defining analytical performance goals" identified three models for defining analytical performance goals in laboratory medicine. Whereas the highest level of model 1 (outcome studies) is difficult to implement, the other levels are more or less based on subjective opinions of experts, with models 2 (based on biological variation) and 3 (defined by the state-of-the-art) being more objective. A working group of the German Society of Clinical Chemistry and Laboratory Medicine (DGKL) proposes a combination of models 2 and 3 to overcome some disadvantages inherent to both models. In the new model, the permissible imprecision is not defined as a constant proportion of biological variation but by a non-linear relationship between permissible analytical and biological variation. Furthermore, the permissible imprecision is referred to the target quantity value. The biological variation is derived from the reference interval, if appropriate, after logarithmic transformation of the reference limits.

A major problem with the use of serum prostate-specific antigen (PSA) in predicting prostate cancer risk is the considerable variability of such measurements. Cramer et al. identified a set of singlenucleotide polymorphisms (SNPs) in the upstream regulatory region of the PSA gene that were each associated with increased promoter activity and serum PSA, further suggesting that genotyping these SNPs could be useful in improving the predictive value of PSA screening. In order to replicate this finding, DNA samples from 475 African-American men were genotyped for the same SNPs and no association was observed with either serum PSA level or prostate cancer diagnosis.

ÖzetGiriş: Total PSA (tPSA)'dan farklı bir metabolizmaya sahip olan serbest PSA (sPSA)'nın, tPSA'dan farklı bir günlük varyasyonunun olup olmadığını; varsa, serbest/total PSA (s/tPSA) oranının gün içinde değişip değişmediğini araştırmak istedik. Materyal ve Metod: Yaş ortalaması 61( 40-84 arası) olan ve üroloji dışında diğer kliniklerde yatan 33 erkek hastadan 8 saat arayla 3 kez kan alınarak aynı anda serbest ve total PSA değerlerine bakıldı. Her üç ölçüm için s/t PSA değerleri hesaplandı ve sonuçlar istatistik olarak değerlendirildi. Bulgular: 33 hastanın %64'ünde tPSA gün içinde gittikçe düşerken bu esnada ölçülen sPSA değerleri önce yükselip sonra hafif azalmış; sabah %20 olan s/t PSA oranı öğleden sonra %22 ve gece %23 olarak hesaplanmıştır. İkinci ve üçüncü hesaplamaların ilkine göre anlamlı (p<0.05) farklılığı olduğu görülmüştür. Tüm hastalarda ise ortalama tPSA'nın gün boyu bir düşüş gösterdiği, buna karşılık sPSA'nın önce düşüp sonra yükseldiği saptanmış; böylece s/t PSA oranında sabah ile ikinci ölçüm arasında anlamlı bir fark olmamasına karşın (p=0.575), sabah ile üçüncü ölçüm ve ikinci ile üçüncü ölçüm arasında anlamlı farklar (p= 0.046 ve p=0.036) olduğu görülmüştür. Sonuç: Prostat kanseri yönünden şüphe duyulan ve s/tPSA oranına bakılan hastalarda, bu oranın gün içinde değişebileceği ve özellikle sınır vakalarda biyopsiye gidişi etkileyebileceği akılda tutulmalıdır. AbstractPurpose: To investigate if there is a different daily variation of freePSA(fPSA) other than tPSA and if any, to exhibit an ultimate fPSA/ tPSA ratio fluctuations that might influence a desicion of biopsy for borderline f/t PSA values. Material and Method: Blood samples were drawn from 33 men with a mean age of 61 years ( 40-84) hospitalized at departments other than urology with different diagnoses, three times a day; morning, afternoon and midnight, with 8-hour intervals, and assayed for fPSA and tPSA. Then f/tPSA calculations were made and the data were analyzed statistically. Results: While in 64% of the patients tPSA decreased all the day, fPSA increased in the afternoon measures and slightly decreased later. f/tPSA value changed from 20% in the morning to 22% and 23% in the afternoon and midnight respectively. The difference between the first and the second calculations were significant and between the first and the third as well (p<0. 05). For all patients, mean tPSA decreased all the day whereas the mean fPSA value decreased similar to tPSA in the second determination but increased later. So the ratio of f/tPSA increased constantly all the day. Although the difference was not significant between morning and afternoon (p=0.575), but it was significant between morning and midnight (p<0.046) and between afternoon and midnight (p=0.036). Conclusion:We conclude that fPSA may have a different daily variation even if slightly in respect to tPSA, and f/tPSA ratio may have a constant increment that might influence a decision for prostate biyopsy in borderline cases.
