Objectives: The pandemic caused by coronavirus disease 2019 (COVID-19) has exerted an unprecedented impact on the health of populations worldwide. However, the adverse health consequences of the pandemic in terms of infection and mortality rates have varied across countries. In this study, we investigate whether COVID-19 mortality rates across a group of developed nations are associated with characteristics of their healthcare systems, beyond the differential policy responses in those countries.Methods: To achieve the study objective, we distinguished healthcare systems based on the extent of healthcare decommodification. Using available daily data from 2020, 2021, and 2022, we applied quantile regression with non-additive fixed effects to estimate mortality rates across quantiles. Our analysis began prior to vaccine development (in 2020) and continued after the vaccines were introduced (throughout 2021 and part of 2022).Results: The findings indicate that higher testing rates, coupled with more stringent containment and public health measures, had a significant negative impact on the death rate in both pre-vaccination and post-vaccination models. The data from the post-vaccination model demonstrate that higher vaccination rates were associated with significant decreases in fatalities. Additionally, our research indicates that countries with healthcare systems characterized by high and medium levels of decommodification experienced lower mortality rates than those with healthcare systems involving low decommodification.Conclusions: The results of this study indicate that stronger public health infrastructure and more inclusive social protections have mitigated the severity of the pandemic’s adverse health impacts, more so than emergency containment measures and social restrictions.

Transportation systems play a pivotal role in facilitating access to out-of-home activities, enabling participation in various aspects of social life. But because of budgetary and physical limitations, they cannot provide equal access everywhere; inevitably, some locations will be better served than others. This realization gives rise to two fundamental concerns in transportation equity research and practice: 1) accessibility inequality and 2) accessibility poverty. Accessibility inequalities may rise to the level of injustice when some socioeconomic groups systematically have lower access to opportunities than others. Accessibility poverty occurs when people are unable to meet their daily needs and live a dignified, fulfilling life because of a lack of access to essential services and opportunities. In this paper, we review two of the most widely used approaches for evaluating transport justice concerns related to accessibility inequality and accessibility poverty: Gini coefficients/Lorenz curves and needs-gap/transit desert approaches, respectively. We discuss how their theoretical underpinnings are inconsistent with egalitarian and sufficientarian concerns in transport justice and show how the underlying assumptions of these methods and their applications found in the transportation equity literature embody many previously unacknowledged limitations that severely limit their utility. We substantiate these concerns by analysing the equity impacts of Covid-19-related service cuts undertaken in Washington, D.C. during 2020. The paper also discusses how alternative methods for measuring transportation equity both better comport with the known impacts of such changes and are consistent with underlying moral concerns.

With the rapid growth of aging populations in South Korea, it is important to assess spatial accessibility to healthcare resources as older adults may need frequent visits to hospitals. Healthcare spatial accessibility is measured based on available resources (e.g., physicians, beds, services), demands (e.g., population), and travel costs (e.g., distance or time). In this study, we employed an Enhanced Two-Step Floating Catchment Area (E2SFCA) method to measure the spatial accessibility to primary care for older populations (i.e., aged 65 and older) in major cities in South Korea, including Seoul, Busan, Daegu, Incheon, Gwangju, Daejeon, and Ulsan. We found that the aging population in Seoul, the capital and biggest city in South Korea, has relatively better accessibility than those living in other cities. We also discovered a negative relationship between accessibility to primary care and the aging index (i.e., population over 65 years old/population less than 15 years old); the regions with a higher ratio of older populations have lower accessibility to primary care. The results suggested that more primary care services (perhaps via mobile vans) are needed in regions predominantly with older people to improve their healthcare access.
