Abstract:要旨【目的】重症外傷患者では血液凝固系の異常を来し,受傷直後からD–dimerが高値で推移するため,血栓の指標となるD–dimerの基準値は確立していない。そこで我々は,静脈血栓塞栓症(venous thromboembolism: VTE)を早期に発見するため,外傷患者においてD–dimerを用いたスクリーニング基準の有用性を検討した。【対象】2011年から2015年までの間に当センターに入院となった外傷患者455例を対象とした。VTE予防として,入院時より可能な限り全例に対して間欠的空気圧迫法を施行した。当センターのVTEスクリーニング基準は「受傷後5日目以降において,D–dimerが3測定日連続して増加,かつ15μg/mL以上」と設定した。スクリーニング基準を満たした症例に対して造影CTを施行し,VTEの有無を評価した。【結果】観察期間中108例がスクリーニング基準を満たし,そのうち,延べ73例で造影CTが施行され,34例をVTEと診断した(陽性率46.6%)。スクリーニング基準は在院7日(中央値)で満たし,造影CTは10日で施行された。診断時,全例でVTEによる症候を認めなかった。一方,基準を満たさなかった… Show more
