Cytoplasmic Fibrils in Plasma Cell Leukemia

Abstract: In a patient with plasma cell leukemia, associated with pleural effusion, ultrastructural studies of the peripheral blood plasma cells showed an abundance of cytoplasmic fibrils. The nature of fibrils was not clarified, but they were not amyloid fibrils. This finding, coupled with a liaterature review, suggests that the cytoplasmic fibrils in plasma cells may be an additional cytological feature of this rare form of leukemia.

“…A striking finding was the presence of abundant cytoplasmic fibrils in the majority of cells. These structures have been described in malignant plasma cells from multiple myeloma (20) but may occur with a higher frequency in PCL (6,21). The nature of these structures remains uncertain.…”

“…Pleural effusion in the absence of intrathoracic plasmocytoma has been ob served in only 6 patients, as reported in the French literature [18,19], in I of these the pleural effusion appeared concomitantly with pulmonary infection and receded fol lowing antibiotic therapy. Two additional patients with MM and pleural involvement [18,19] as well as 2 patients with plasma cell leukemia complicated by pleural effu sion have also been recently described [16,17].…”
