Cytomegalovirus (CMV) shedding is highly correlated with markers of immunosuppression in CMV-seropositive women

Abstract: Cytomegalovirus (CMV) enters latency following primary infection and can subsequently reactivate. Reinfection with a different viral strain can also occur. During these events, CMV is shed in bodily fluids. This study examined correlates of CMV shedding in specimens obtained from the HIV Epidemiology Research Study, a multicenter cohort study of US women with or at high risk for human immunodeficiency virus (HIV) infection. Among the women studied, 91.4 % (911/997) were CMV IgG seropositive. Of these women, 2.… Show more

“…HIV-infected women are often CMV seropositive and experience more frequent CMV recurrences with progressive immune impairment (78)(79)(80). Studies in Europe and the Americas support an increased risk for congenital CMV infection in neonates born to HIV-CMV-coinfected mothers (79,81,82).…”

“…[26][27][28][29][30] Others have reported detectable levels of CMV DNA in CVL, saliva, and peripheral blood mononuclear cells (PBMCs). 25,31,32 However, immunological markers of CMV infection in blood specimens have shown significant associations with biomarkers of inflammation and non-AIDSdefining events. Therefore, quantitative measures of the immune response to CMV have been used to establish associations with biomarkers of inflammation and disease conditions in HIV-1 patients.…”

“…Most studies in which HIV-infected mothers were not immunosuppressed found no difference in congenital CMV infection rates [10, 18], whereas those with immunosuppressed women found increased congenital CMV infection in HIV-exposed infants. This is biologically plausible, as immunosuppression correlates with CMV shedding in the genital tract of HIV-infected women [14, 17, 29, 33]. Additionally, emerging evidence suggests that maternal ART may decrease the rate of congenital CMV infection [12, 20, 2325], likely by improving maternal health and immunity, resulting in decreased mucosal shedding of CMV during birth and a reduced incidence of reinfection or disease reactivation.…”
