Objective: With regard to the correlation between T helper1/T helper2 (Th1/Th2) cell balance and 1α,25-dihydroxyvitamin D 3 , active metabolite of vitamin D, we studied Th1/Th2 cell balance by measuring levels of the cytokines interleukin (IL)-4, IL-10 and interferon-gamma (IFN-γ), which are important for immune response of patients with allergic rhinitis. Methods:Thirty adult patients diagnosed with allergic rhinitis (study group) and 40 healthy volunteers (control group) are examined in the research. IFN-γ, IL-4, IL-10, and total immunoglobulin E (IgE) levels from serum samples and vitamin D 3 levels from plasma were determined in all patients. Results:In IgE, IL-4, IL-10, IFN-γ, and 1α,25-dihydroxyvitamin D 3 levels (p<0.05), a statistically noticeable difference was observed between the study and control group. The 1α,25-dihydroxyvitamin D 3 levels in both groups were compared and a statistically significant difference between the 1α,25-dihydroxyvitamin D 3 levels in the study group and that in the control group (p<0.05) was observed. There was a positive correlation between IFN-γ and vitamin D levels (p<0.05) in the study group, whereas IgE, IL-4, and IL-10 levels showed a negative correlation with vitamin D 3 levels (p<0.05). Conclusion:In our study, 1α,25-dihydroxyvitamin D 3 levels were associated with Th1/Th2 balance in allergic rhinitis, and a remarkable correlation was observed among vitamin D deficiency and allergy. These findings show that 1α,25-dihydroxyvitamin D 3 may have a remarkable role in the severity and control of allergic disorders. In addition, further investigations are required to confirm how vitamin D should be used in allergic diseases. Corticotropin-releasing hormone (CRH) is secreted under stress and regulates the hypothalamic-pituitary-adrenal (HPA) axis; it is also secreted outside the brain where it exerts proinflammatory effects, possibly through mast cell activation. Mast cells are necessary for allergic reactions, but are increasingly implicated in acquired immunity and inflammatory diseases worsened by stress. Acute stress and intradermal CRH induced murine skin mast cell activation and increased vascular permeability that was absent in W/W v mast cell deficient mice. The presence of functional CRH receptors (CRH-R) was recently reported on human mast cells. Here, we studied the expression of CRH-R1 and CRH-R2 by semiquantitative reverse transcriptase-polymerase chain reaction (RT-PCR) and fluorescent immunocytochemistry in human umbilical cord blood-derived cultured mast cells (hCBMCs) treated with Interleukin (IL)-1, IL-4 or lipopolysaccharide (LPS). Ten week-old hCBMCs cultured in the presence of Stem cell factor (SCF) and IL-6 were positive for both CRH-R1 and CRH-R2. However, the expression of only CRH-R2 mRNA and protein was induced by priming hCBMCs with IL-4 for the last three weeks of culture. Further analysis of the CR-H R2 mRNA expression showed that addition of IL-1 or LPS for 6 h increased only CRH-R2 gene expression. CRH had negligible effect on IL-6 secretion from non-primed hCBMCs, but induced release from IL-4 primed cells. Interestingly, LPS alone increased IL-6 release in non-primed cells, but lost this effect in primed cells. These results further implicate mast cells and CRH in either initiating or potentiating inflammatory diseases, especially those affected by stress. The major birch allergen Betulae verrucosa 1, (Bet v 1), belongs to the proteinase 10 (PR-10) family of panallergens that are common to many fruits, nuts and vegetables. A high proportion of birch-sensitised individuals experience oral symptoms upon consumption of such foods. This has been termed 'oral allergy syndrome (OAS),' or 'pollen-food syndrome.' Birch pollen specific immunotherapy (BP-SIT) can successfully treat birchsensitive rhinitis; it has been postulated that BP-SIT might also reduce oral allergy symptoms. Previous studies have been small and contradictory, using differing methodology and primary outcome measures.We designed a placebo controlled, double blind, randomised study aiming to establish definitively whether BP-SIT can effectively treat the pollen-food However, a total of eight patients were noted to have an apple threshold of 100g or more at baseline, despite reacting to only 20g during screening. The clinical trial is on going and remains blinded; it is therefore impossible to draw any firm conclusions regarding the efficacy of BP-SIT to treat OAS at this time.However, the study has raised questions concerning the validity of DBPCFC as the gold standard test in oral allergy syndrome, something not previously reported.II II
