Cytodiagnosis of Gouty Arthritis

Abstract: Gout is a disorder of uric acid metabolism that causes deposition of Monosodium Urate (MSU) crystals in joint spaces, which, in turn, elicit an acute inflammatory reaction. We present a case in which diagnosis of gout was made by fine needle aspiration of tophus. FNAC of gouty tophus is an easy alternative to synovial biopsy. It is simpler, easier and less painful.

“…Aspirate in gouty tophus is chalky white and on smears, there is a presence of slender needles-shaped crystals whereas the crystals in pseudogout are smaller, rhomboid in shape. The crystals in pseudogout are due to deposition of calcium pyrophosphate dihydrate (CPPD).Also, the gout crystals are negatively birefringent on polarized microscopy and CPPD crystals are weakly positive [4].…”

“…The definitive diagnosis of gout is best established by demonstration of monosodium urate crystals . [1] The most common site is first metatarsophalangeal joint space. Fine needle aspiration cytology (FNAC) is simple, cost effective, safe procedure in diagnosing periarticular nodules.…”
