ImportanceIntradetrusor injection of onabotulinumtoxinA is performed via varying injection paradigms but no studies have studied the various effects of these paradigms on patient experience with the procedure.ObjectivesThis randomized clinical trial aims to compare pain and procedure time between patients receiving a 100-unit dose of onabotulinumtoxinA in 5 injections compared to 20 injections for the treatment of idiopathic overactive bladder or urgency urinary incontinence.Study DesignPatients presenting with refractory overactive bladder or urgency urinary incontinence at 2 clinical sites were identified and randomized to undergo onabotulinumtoxinA treatment with 5 injections versus 20 injections. Patients rated their pain level on a 10-point visual analog scale at procedure completion. The procedure duration was recorded with a stopwatch. Patients were followed up 6 weeks postprocedure, at which time they completed a Global Response Assessment to determine subjective efficacy of treatment. Participants were additionally monitored for incidence of adverse events in the follow-up period.ResultsThe average pain score was not statistically significant between groups (2; interquartile range, 1–4 for the 5 injection group vs 3; interquartile range, 2–4 for the 20 injection group; P = 0.27). Patients who received 5 injections experienced significantly shorter mean procedure time as compared with patients who received 20 injections (76 seconds vs 176 seconds; P < 0.001). There were no differences in subjective efficacy or adverse events between groups.ConclusionsPerceived pain, efficacy, and postprocedure complications did not significantly differ between patients receiving 5 injections and 20 injections, but procedure time was significantly shorter.

Aims: To study the pattern of LUTS during diagnostic cystoscopy and to evaluate patient’s satisfaction using rigid cystoscopy with intra-urethral lignocaine gel. Methods: Hospital based cross-sectional observational study done in all patients attending Department of Obstetrics and Gynecology with LUTS. Diagnostic cystoscopy was performed after using intra-urethral lignocaine gel. Patients profile and indications for cystoscopy were recorded; and VAS (Visual Analogue Scale) was used to assess procedure related pain. Data stored in MS Excel and descriptive analysis performed using SPSS 21. Result: Forty-three patients were included in the study. The mean age of presentation was 45.77 years. Majority of the patients were pre-menstrual and with symptoms less than 5 year duration. Twenty-seven (62.8%) patients had urgency as indication for cystoscopy followed by cystitis and recurrent urinary infection each (16.3%). Only 2 patients had mixed urinary incontinence. Thirty- one (72.1%) had mild discomfort, seven (18.7%) moderate and 5(9.2%)severe in visual analogue scale. Conclusions: Urgency was found to be a frequent indication for diagnostic cystoscopy and use of rigid cystoscope using inra-urethral gel showed only mild discomfort to majority of patients.

Impact of Preoperative Video-Based Education on Anxiety Levels in Patients with Renal Stones Scheduled for Flexible Ureteroscopic Lithotripsy: A Comparative Study Using APAIS and STAI

Background Anxiety prior to surgery can impact patient outcomes and satisfaction. In light of this, our study aimed to examine the effects of preoperative video-based information versus verbal information on anxiety levels among 60 patients with renal stones slated for flexible ureteroscopic lithotripsy. Anxiety was measured using the Amsterdam Pre-Operative Anxiety and Information Scale (APAIS) and the State-Trait Anxiety Inventory (STAI). Material/Methods We enrolled 60 patients with renal stones who were scheduled for flexible ureteroscopic lithotripsy. These patients were divided randomly into two groups: Group 1 (30 patients) received preoperative video-based information and Group 2 (30 patients) received only verbal information. Anxiety levels were assessed preoperatively using both the APAIS and STAI. On the first postoperative day, each patient indicated their pain using a visual analogue scale and filled out a questionnaire about their willingness to undergo the procedure again. Results Group 1 exhibited lower anxiety levels as reflected by the APAIS scores for anesthesia ( P =0.02), surgery ( P <0.001), overall ( P <0.001), and information needs ( P <0.001). While there was no significant difference in the STAI-trait anxiety scores between the groups ( P =0.15), Group 2 demonstrated notably higher anxiety levels concerning the surgical procedure, as measured by the STAI-state scores ( P <0.001). The average pain score was 4.23±2.17 for Group 1 and 5.37±2.83 for Group 2 ( P =0.08). Patients in Group 1 showed a greater willingness to undergo the surgery again ( P =0.02). Conclusions Delivering preoperative information through videos significantly diminishes anxiety levels in patients about to undergo retrograde intrarenal surgery compared to traditional verbal communication.
