Cystosarcoma phyllodes of the seminal vesicle

Abstract: Background : We report here an extremely rare case of cystosarcoma phyllodes of the seminal vesicle. Methods : A 65-year-old man presented with urinary hesitancy, frequency and constipation. Clinical examinations including two needle biopsies were performed, and the patient had undergone open surgery. Results : The final pathological diagnosis was cystosarcoma phyllodes of the seminal vesicle. “…Rectal examination, TRUS, CT scan and MRI were valuable for locating the mass. The experience provided by the present case, together with that of Khan et al 7 and Abe et al, 6 suggests that biopsy may not be helpful in confirming the nature of the tumour. Excision of the tumour and vesiculectomy is appropriate for tumours with an intact surface as it is less invasive and associated with a quicker recovery and higher quality of life.…”

“…The median duration of follow‐up for 32 cases after their first surgical approach was 21.0 months (IQR 11.75–39.0 months). The outcome in most cases was “NED.” However, two cases had local recurrence diagnosed pathologically as low and intermediate grade, respectively, 8,9 and another two had lung metastases within 48 months, diagnosed as high grade 10,11 . One patient with high‐grade disease died 11 months after metastatectomy 10 .…”

“…They proposed that like breast and prostate tumors, phyllodes tumors of the seminal vesicle should be considered high-grade lesions (malignant) with significant mitotic activity, stromal pleomorphism, and stromal overgrowth [8]. Abe et al reported a patient suffering from cystosarcoma phyllodes of the seminal vesicle who died 11 months after tumor removal by open operation [9]. The selection of the initial surgical approach is of great importance for phyllodes tumors of the breast.…”
