PurposeDifferent techniques for cystocele repair including the conventional anterior colporrhaphy and mesh technique are known. Our goal was to evaluate the anatomical success and safety of our method of transvaginal anterior vaginal wall repair by the purse-string technique reinforced with three simple additional sutures in the repair of cystocele over a 4-year follow-up period.MethodsThis was a retrospective review of 69 consecutive patients (grades 2 to 4) who underwent the above operations between 2001 and 2011, including their success rates as assessed by use of the Baden-Walker halfway classification system.ResultsOf the patients, 62 patients (98%) were completely cured of cystocele and 1 patient showed grade 2 cystocele recurrence that required no further treatment. Two patients with grade 4 cystocele were completely cured. There was no vaginal erosion related to the cystocele repair.ConclusionsTransvaginal anterior colporrhaphy by a purse-string technique reinforced with simple additive sutures appears to be a simple, safe, and easily performed approach in cystocele repair. There is no need for other material for reinforcement, even in high-grade cystocele, which is an advantage of our technique.

Background The use of large vaginal meshes for the treatment of pelvic organ prolapse (POP) in combination with poor surgical experience are the dominating risk factors for serious postoperative complications. The main goal of this study is not only to select a safe and effective method of POP surgery, but also to form a standardized teamwork algorithm aimed at diagnosing, treating and postoperative management of patients with subsequent monitoring of their condition. Methods A retrospective study of 5031 medical history was used to evaluate the effectiveness of the quality improvement intervention. We evaluated the following indicators: type of surgery performed, duration of surgery, duration of postoperative bed-day. Interventions From 2012 to 2019, seven types of surgical techniques with the use of the mesh material were consistently used, differing in the volume of the implanted mesh material, the presence and type of simultaneous native tissue reconstructive surgery, the method of mesh fixation and the number of fixation points. Results Patients who underwent technique number “7” showed the highest result in the mean scores (33±15 scores PISQ12, 51±30 scores PFDI20) of the questionnaires, which was statistically significant in comparison with other techniques (p<0,001). The number of postoperative complications were statistically significantly higher in techniques “1” to “6” compared to “7” (p<0.001). There was no difference in supportation rate between all techniques (p=0.12). Resultados: A idade média foi 60 anos (DP=10,3); 76% apresentaram nível educacional básico ou médio, com companheiro (61%), com atividade sexual (40%), com menopausa (74,5%); 2,5 procedimentos cirúrgicos para correção do POP em média. A QVRS melhorou após o tratamento cirúrgico, em ambos os seguimentos (p<0,001), e em todas as dimensões. A imagem corporal apresentou diferença significativa (p<0,05) em todos os momentos de avaliação: para cada ponto de piora da imagen corporal, menor mudança na QVRS após a cirurgia. Os sintomas menopáusicos impactaram em menores mudanças na QVRS nas dimensões PGS e E, em todos os tempos, embora a QVRS nas dimensões IP, LF, LS e SE tenha apresentado leve porém significativa melhora após a cirurgia (p<0,05). As mulheres com doença neurológica mostraram menores mudanças na QVRS no pós-operatório comparativamente àquelas sem essa doença nas dimensões LF, E e SE (p<0,05). Outras variáveis preditoras de QVRS foram a atividade sexual, nível educacional, idade e diabetes mellitus. Obteve-se um efeito de grande magnitude em todas as dimensões e estabeleceu-se MID entre-1,19 e-15 pontos para todas as dimensões de PQOL. Conclusão: A QVRS melhorou após o tratamento cirúrgico, em todas as dimensões, e as variáveis preditoras mais importantes foram a imagen corporal, os sintomas menopáusicos e as doenças neurológicas. O estudo contribui para corroborar a necessidade de avaliação integral das mulheres no pré-operatório visando a conhecer as tendências de sua QVRS posteriormente atuando nos fatores modificáveis que podem influir negativamente em sua recuperação pós-operatória.
