Cystic pancreatic endocrine tumors

Abstract: BACKGROUND: Cystic pancreatic endocrine tumors (PETs) are rare neoplasms with a preoperative diagnostic challenge. The aim of this study was to evaluate the preoperative diagnostic strategy for these tumors and to assess the clinical and pathologic characteristics. METHODS: Six cases of cystic PET were retrospectively enrolled. Endoscopic ultrasound‐guided fine‐needle aspiration biopsy (EUS‐FNAB) was performed in 4 cases. All cytomorphologic data from conventional smears, ThinPrep preparations, and cell block … Show more

“…Other treatment options include radiofrequency ablation, chemoembolisation, radioembolisation, palliative surgery (eg debulking), systemic chemotherapy (for high grade tumours) and medical therapy (eg somatostatin analogues) to reduce symptoms in functional tumours. 4,8,9 It is essential that tumours are graded accurately at initial diagnosis to ensure that patients with low grade tumours are not put through unnecessary treatments, that those with aggressive disease are not undertreated 24 and that those for whom curative therapy exists, receive it.…”

“…In our patient, the cyst fluid levels of carcinoembryonic antigen were normal. Elevated levels of carcinoembryonic antigen in cysts are a useful marker of mucinous cysts [13,14] and it is therefore interesting to note that numerous previous studies have reported that cyst fluid levels of carcinoembryonic antigen are usually low in cystic neuroendocrine tumors [15,16]. …”

“…Imaging is unable to reliably differentiate cystic-NETs from other PCLs as well as simple cysts, pseudocysts or SCAs [ 77 , 82 ]. Thus, EUS with fine needle aspiration and immunostaining has been suggested as a more effective modality for diagnosing cystic-NETs [ 77 , 83 ].…”

“…Cystic-NETs have been shown to stain positive for synaptophysin and chromogranin [ 75 , 77 , 79 , 83 , 84 ]. This staining helps distinguish Cystic-NETs from other PCLs that may have similar histological or immunohistochemical features.…”
