Background: Human cystic echinococcosis is a zoonosis occurring due to dogs handling and exposure to Echinococcus granulosus ova in their stools being an accidental intermediate host. Serology remains the only sure and important tool to diagnose this disease. Our objectives in this research are to improve the serodiagnostic abilities of sandwich ELISA through its binding to gold nanoparticle and proving the role of minute nanoparticles in serodiagnosis. Methods: gold nanoparticles (AuNPs) were employed for the capture of the antigens of protoscolices (pAg) in the patient’s serum by sandwich ELISA. Cross-reactivity for antigens from Hymenolepis nana , Entrobius vermicularis , and Fasciola gigantica was ruled out by using anti-protoscolices polyclonal antibodies (ppAb). (pAg) of sonicated protoscolices which were removed from camel lung cysts, was purified by diethyl aminoethyl Sephadex (DEAE-Sephadex) and injected to a New zealand white rabbit giving ppAb which then conjugated to horseradish peroxidase (HRP) and loaded on AuNPs being used as a diagnostic indicator for circulating pAg by both sandwich ELISA and nanogold sandwich ELISA techniques. Results: Nanogold sandwich ELISA was able to give positive results with 96.3% of hydatid patients and 5% of non-hydatid patients while sandwich ELISA showed 81.4% and 20% positive cases of the same groups respectively. The sensitivity and specificity of nanogold sandwich ELISA were 96.3% & 95% against 81.5% & 80% for those of sandwich ELISA respectively. Conclusion: The conjugation of AuNPs to anti- Echinococcus IgG antibodies seems to be beneficial to maximize the sensitivity of sandwich ELISA, reduce the overall cost of the assay as less antibody was needed, solve harsh buffering conditions and achieve greater stability which provides a reliable quantitation of analytes.

Objective: The aim is to determine the effectiveness of indirect Haemagglutination (IHA) as diagnostic techniques for Cystic Echinococcosis in Iraqi and the possible correlation of age and gender in disease occurrence. Material and Methods: IHA technique using sensitized sheep red blood cells coated with an Echinococcus granulosus antigen. A chi square, Pearson correlation used for analysis of categorical variables. Results: Patients presented with clinical manifestation of hydatidosis (202) selectively enrolled in this study. The mean age was 31.64±8.98 years. The minimum age was 18 years and the maximum age was 70 years. Males represent 50%, with the mean age of 33.50±7. 8 years. The rest of patients were females with a mean age of 29.78±9. 6 years. Positive cases represent 19.3%, and the 80.7% were negative. The number of positive cases was higher among females (22.8%) compared with males (15.8%) with no significant difference (p= 0.212) or correlation between gender and IHA results (p = 0.214) .The majority of patients had a the rage of 24-30 years, 31-36 years and 18-23 years. Among males, a higher number of positive cases at 31-36 years followed by 24-30 years and 49-54 years. Among females, a higher number of positive cases with 24-30 years of age was followed by those with 37-42 years of age and 18-23 years of age. There was a significant difference (p= 0.001) between age groups according to gender without a significant correlation between the age group and IHA results according to gender (p= 0.268). Conclusion:The IHA is considered as rapid, reliable and adequate technique that makes it possible to process a large number of samples simultaneously. It does not require highly trained technical personnel. Females appear to be more susceptible to CE than males especially in younger age groups.

Background: Hydatid cysts are one of the serious complications following echinococcus infection. The liver and the lungs are the most affected organs, respectively. The severity of the disease is associated with the increase in the number and the size of the cysts, cysts rupture, and systemic effects. The aim of this study is to evaluate prognostic factors that are associated with the increased incidence of postoperative complications following pulmonary hydatid cyst surgery. Methods: Patients referred to Madani hospital from 2014-2018, presenting pulmonary hydatid cysts were included in this study. All the patients were evaluated based on the following parameters: age, gender, location and size of the cysts, rupture status of the cysts (intact or perforated), type of surgical intervention (capitonnage or segmentectomy) and Erythrocyte Sedimentation Rate (ESR). The factors were then compared with postoperative complications. Statistical analysis of the data obtained was conducted using R-software Results: Of 76 patients enrolled in our study, 52.63% were males and 47.36% were female. Air leak complication was reported in 13.15% of the patients and 3.94% of the patients were presented with pleural effusion. Postoperative complications were significantly associated with the perforated (ruptured) cysts p= 0.001, segmentectomy p= 0.013, giant hydatid cysts p= 0.007 and ESR p= 0.014. However, the side of the lung was not significantly related to postoperative complications. Conclusion: Our study reports that perforated cysts, increased size, segmentectomy and abnormal ESR are likely to increase postoperative complications following pulmonary hydatid cysts surgery. Prospective studies with perioperative parameters and greater sample size can help to deduce better inferences.
