Cystic cavernous hemangioma of the liver

Abstract: We report an unusual case of multilocular cystic cavernous hemangioma of the liver. The patient was a 61-year-old woman without liver disfunction but who had multicystic mass lesions in the liver. Although cavernous hemangiomas are usually accurately diagnosed by the various imaging modalities, our case showed atypical features.

“…Such pathological characteristics make radiological diagnosis using enhanced studies very difficult, especially from hypo‐vascular adenocarcinomas, including cholangiocarcinomas, metastatic liver cancers or fibrolamellar hepatocellular carcinomas 2–10 . Enhanced CT may show peripheral, globular enhancement of the lesion at the early phase, centripetal enhancement that progresses to uniform filling on the portal venous phase, with this enhancement persisting in the delayed phase 2–12 …”

“…All of these findings in the imaging studies might have been dependent on the rich fibrous ingredients, and it is difficult to differentiate hepatic sclerosed hemangioma from other disease entities, including cholangiocarcinoma and colorectal metastasis. Moreover, there are also other types of hepatic cavernous hemangiomas including calcified hemangioma, hyalinized hemangioma, multilocular cystic hemangioma, pedunculated hemangioma, hemangioma with arterial‐portal venous shunt, hemangioma with capsular retraction, or nodular hyperplasia surrounding hemangioma 10–13 . These other types of hemangiomas also make the imaging diagnosis difficult.…”

“…In some cases, hemangiomas present with peculiar characteristics. Some lesions may present cystic degeneration ( Figure 5), many times centrally located, varying in extent, and, in some situations present like a lesion containing a significant cystic component (cystic hemangioma) (8) . Areas of necrosis also may be observed, as well as central fibrotic areas.…”
