Cyclosporine A up‐regulates Sonic hedgehog in gingiva: role of the up‐regulation on gingival cell proliferation

Abstract: Cyclosporine A up-regulates gingival SHH expression in vitro and in vivo, and the inhibition of the SHH pathway counteracts the stimulatory effect of cyclosporine A on gingival fibroblast proliferation. Therefore, we suggest that SHH mediates a novel molecular mechanism for cyclosporine A-induced gingival complications.

“…Furthermore, the low dose of steroids universally used in KT is expected to reduce skin thickness rather than increase it. Interestingly, cyclosporine has been shown to increase sonic hedgehog (SHH) signaling that, in part, drives gingival hyperplasia (14,15). Hedgehog signaling is also a key player in mediating the inflammatory response in BCC (2).…”

“…Among the off-target effects of CNI are the reduction of pruritus and erythema 38 excessive hair growth, acne, and, rarely, gum hyperplasia have been reported. 39 The latter might be due to the ability of CNI to increase sonic hedgehog (SHH) which is linked to gingival hyperplasia 40,41 and to the inflammatory response in BCC. 33 A 2010 study has shown that keratinocytes infected by H-rasV12-transducing retrovirus more readily progress to malignancy when CNI are used.…”
