Cyclosporine for lupus membranous nephritis:experience with ten patients and review of the literature

Abstract: Proteinuria and serum albumin improved in all patients on CYS. A literature review is consistent with this. Controlled studies of the use of CYS for membranous lupus nephritis would be useful.

“…The committee defined improved as a 50% reduction in the urinary protein: urinary creatinine ratio. The literature is replete with definitions of improvement based on the 24-hour urine protein excretion level (5,9,15,16,18,24,32,34,35,38,47,52,66,72,74,75,80,81,(83)(84)(85)(86)(87)(88)(89)(90)(91)(92)(93)(94)(95)(96)(97)(98). In addition, the committee thought that a reduction in proteinuria from the nephrotic range (Ն3.5 gm/day) to the non-nephrotic range was an important and distinctive end point.…”

“…Other papers reported that cyclosporine induced remission in most patients with SLE membranous nephropathy, but relapse of proteinuria was frequent when the drug was withdrawn (12,13). Clinical and histologic improvement with cyclosporine has been reported also in proliferative lupus nephritis both when the drug was given in patients resistant to IV cyclophosphamide (14) or when given for maintenance (15).…”

“…Ob die beiden letzteren Medikamente, bezogen auf die Wirksamkeit, als gleichwertig anzusehen sind, ist unbekannt, da keine prospektiven randomisierten Studien zum Vergleich vorliegen. Es liegen jedoch aus mehreren nicht kontrollierten Studien ausreichende Ergebnisse aus dem Erwachsenenalter vor, die eine Empfehlung zum Einsatz eines der beiden Medikamente bei Klasse-V-LN bekräftigen [39,40,41]. …”
