AIM:This study aimed to observe the cyclooxygenase-2 expression and its correlation with tumour size and lymph node involvement in nasopharyngeal carcinoma.METHODS:This study was cross-sectional, that enrolled 126 samples diagnosed with nasopharyngeal carcinoma in Haji Adam Malik General Hospital, Medan, Indonesia which fulfilled the inclusion criteria.RESULTS:Based on this study, we found that the age peak incidence of nasopharyngeal carcinoma patients about a 41-60-year-old group (57.1%), dominated by men (71.4%). Through histopathological examination, non-keratinizing squamous cell carcinoma is the most predominant type (79.4%). We also found T3 is the most prevalent primary tumour size (32.5%) with prominent lymph node involvement N3 (45.2%), and stage IV (54.8%). Cyclooxygenase-2 overexpression is prevalent among nonkeratinizing squamous cell carcinoma (81.1%), T3 primary tumour size (41.1%), N3 node involvement (60.0%), and IV clinical stage (71.6%). In addition, we found a significant relationship between cyclooxygenase-2 expressions towards tumor size (p < 0.001) and lymph node involvement (p < 0.001) in nasopharyngeal carcinoma.CONCLUSION:It is proved that the overexpression of cyclooxygenase-2 will increase the susceptibility of nasopharyngeal carcinoma patients having advanced primary tumour size and lymph node involvement.

Testicular degeneration is a multifactorial process increasing the concentration of prostaglandins in seminal plasma. Both increase and decrease of these hormones tend to promote loss of seminal quality. Flunixin meglumine is a potent anti-inflammatory drug capable of modulating the production of prostaglandins and is widely used in female reproduction. However, it is rarely used in males with the same objective. Therefore, this study aimed to evaluate the effects of this drug on seminal quality of male lambs and goats with spermatic characteristics unfavorable for reproduction. To this end, a total of 15 breeding animals were evaluated, of which six goats and four sheep with poor seminal quality were selected according to the criteria established by the Brazilian College of Animal Reproduction (CBRA). Three semen samples were collected from each animal. Then, the flunixin meglumine treatment was initiated and ejaculates were collected at two different periods after the drug was administered (from day 21 until day 35 and from day 49 to day 63). Macroscopic and microscopic parameters were assessed in semen samples and scrotal circumference and percentages of sperm pathologies were measured and compared between the three periods. Data with normal distribution were analyzed using ANOVA at 5% probability, and comparisons between periods within the same species were performed using the Tukey test. An improvement was observed in the analyses of mass motility, percentage motility, and sperm vigor. Scrotal circumference had no variation. Concerning sperm pathologies, an increase in the number of normal spermatozoids was observed due to a significant reduction in minor and major defects, and the latter remained low even after the treatment was finished. Therefore, flunixin meglumine presented beneficial effects on seminal parameters of male goats and lamb with unfavorable spermatic characteristics. These findings indicate this drug may be used in the treating of males with testicular injuries or of breeders with reduced seminal quality due to advanced age. Key words: Andrology. Elevated HSP90 associates with expression of HIF‐1α and p‐AKT and is predictive of poor prognosis in nasopharyngeal carcinoma

Aims HSP90, as a molecular chaperone, has numerous substrate proteins, including HIF‐1α and p‐AKT, but the relationships among HSP90, HIF‐1α and p‐AKT have not been investigated in NPC. We examined and analysed the correlation between expression of HSP90, HIF‐1α and p‐AKT and clinicopathological features of NPC. Methods We collected 445 cases of NPC and 54 cases of non‐cancerous nasopharyngeal epithelia tissues, detected expression of HSP90, HIF‐1α and p‐AKT proteins in these tissues by immunohistochemistry. Results The results indicated that overexpression of HSP90, HIF‐1α and p‐AKT in NPC was significantly higher than that in non‐cancerous nasopharyngeal epithelia (P < 0.05). The overexpression of HIF‐1α in primary NPC was significantly lower than that in matched lymph node metastatic NPC (P = 0.024) or recurrent NPC (P = 0.039). The overexpression of HSP90 (P < 0.001) and HIF‐1α (P = 0.031) was evidently higher in late stage NPC. NPC patients with lymph node metastasis (LNM) had a higher overexpression rate of HSP90 (P < 0.001) than those without LNM. Increased HSP90 expression was positively associated with HIF‐1α expression (r = 0.367, P < 0.001) and p‐AKT (r = 0.142, P = 0.003) expression in NPC. Furthermore, HIF‐1α was also related to p‐AKT expression (r = 0.114, P = 0.017). The overall survival rate for NPC patients with up‐regulated HSP90 was significantly lower than those with down‐regulated HSP90 (P < 0.001), as was found with raised HIF‐1α (P = 0.036) and increased p‐AKT (P = 0.044). Multivariate Cox regression analysis further identified that HSP90 and HIF‐1α were independent poor prognostic factors for NPC. Conclusions Taken together, elevated HSP90 was associated with expression of HIF‐1α and p‐AKT in NPC. Furthermore, high expression of HSP90 and HIF‐1α could be used as a novel independent poor prognostic biomarker for patients with NPC.
