Cutaneous Myiasis of the Breast: Mammographic and US Features—Report of Five Cases

Abstract: Five patients with breast myiasis underwent mammography, and three also underwent ultrasonography (US). Mammography revealed indistinct masses in all patients, with associated pairs of microcalcifications in three. US showed each larva as a fusiform hyperechoic mass surrounded by a hypoechoic halo, which included larval movement in one patient. These imaging features of breast myiasis facilitate correct diagnosis.

“…3A) was detected in the proband (PS1); it was located at exon 5 and resulted in the substitution of arginine for glutamine at amino acid position 147 of the BRCA2 protein. This change was also reported as a VUS of which there were seven previous observations in unrelated families reported in BIC database, the majority of which were observed in individuals of nonspecific and Asian ancestry [15,18]. This variant was not detected in donated blood samples of 50 healthy Chinese female individuals (data not shown).…”

“…Besides clear pathogenic mutations, many variants (variant of unknown significance, VUS) are also found [18]. VUS account for approximately half of all unique variants detected [18,19].…”

“…Color Doppler sonography, which is able to visualize the continuous movement of internal fluids of the larva, has proven to be useful for the detection of D. hominis and C. anthropophaga larvae when ultrasound is not able to detect the parasite (279,286). Mammography of breast myiasis may not be able to exclude malignancy (81). Magnetic resonance and computed tomography (CT) are able to identify a subcutaneous segmented nodule; however, its morphology does not aid in the diagnosis (38,43).…”

“…If necessary, the larva may be killed by the obliteration of the breathing hole (with butter, petroleum jelly, or heavy oils). 2,11 Complications, including granuloma formation and scarring, are rare. Secondary infection by bacteria is also uncommon, because bacteriostatic activity in the gut of the larva seems to prevent undesirable overgrowth of pyogenic bacteria.…”
