Background: Cutaneous metastases can occur in a wide variety of internal malignancies and may be the first sign of a clinically silent visceral cancer. Study Design: A retrospective analysis was made of 138 cases diagnosed with cutaneous and subcutaneous metastasis on fine-needle aspiration cytology (FNAC). Primary tumors of the skin/subcutis were excluded. Results: Of 138 cases, the primary was known in 101 cases and unknown in 37 cases. The age of the patients ranged from 5 to 86 years, and 76 (55.1%) were male and 62 (44.9%) were female. Clinically, the most common lesion was a single nodule (n = 77, 55.8%). The chest wall was the predominant site (n = 53, 38.4%). In males and females, the most common primary sites were the lung (n = 16) and breast (n = 24), respectively. On cytology, the most common diagnosis was metastatic adenocarcinoma (n = 41, 29.7%). Of 37 cases with an unknown primary, FNAC helped to locate the primary site in 17 (45.9%) cases, while in 20 cases it remained undiagnosed. Conclusions: FNAC is a rapid and safe technique that can be used as a first line of investigation for confirmation of metastatic lesions of the skin. Critical evaluation of cytomorphological features along with relevant clinical details could help in the localization of an unknown primary site in some cases.

StreSzczenieNowotwory układu pokarmowego należą do najczęściej występujących chorób i są również jedną z najczęstszych nowotworowych przyczyn zgonów na świecie. Obecność charakterystycznych zmian skórnych pozwala na szybsze rozpoznanie i leczenie, a tym samym zwiększenie prawdopodobieństwa wyleczenia. W artykule przedstawiono najważ-niejsze skórne zespoły paraneoplastyczne, które mogą współistnieć z nowotworami przewodu pokarmowego, w tym rakiem jelita grubego, żołądka i przełyku oraz trzustki. Uwzględniono zespoły uwarunkowane genetycznie, takie jak zespół Cowden, zespół rodzinnie występu-jących licznych znamion atypowych -czerniak lub rak trzustki, zespoły Clarke Howela-Evansa, Peutza-Jeghersa, Muira-Torrego, Gardnera, zespoły nabyte, takie jak rogowacenie ciemne złośliwe, zespół tripe palms, Lesera-Trelat, Bazexa, hypertrichosis lanuginosa, erythema gyratum repens, zespół rakowiaka, zespół glukagonoma, a także przerzuty nowotworowe do skóry oraz nowotwory w niektórych przypadkach zapalenia skórno-mięśniowego. AbStrActGastrointestinal tumors are among the most common neoplastic causes of death worldwide. Presence of characteristic skin lesions can allow faster diagnosis and therapy and this way can increase the probability of a cure. In the paper we present the most important paraneoplastic syndromes that can coexist with gastrointestinal malignancy including colon, gastric, esophagus and pancreatic cancers. We take into account genetic syndromes such as Cowden syndrome, familial atypical multiple mole melanoma syndrome (FAMMM) (melanoma/pancreatic cancer), Clarke Howel-Evans, Peutz-Jeghers, Muir-Torre, Gardner syndromes and acquired syndromes such as acantosis nigricans maligna, tripe palms, Leser-Trelat, Bazex, hypertrichosis languinosa, erythema gyratum repens, carcinoid and glucagonoma syndrome. We also include cutaneous metastases and coexistence of neoplasia in some cases of dermatomyositis.

Electrochemotherapy in the Treatment of Massive, Multisite Breast Cancer Metastasis to the Skin and Subcutaneous Tissue: A Case Report

Background: Chest wall recurrence (CWR) from breast cancer after mastectomy is a difficult to treat disease. Electrochemotherapy (ECT) provides a safe, efficient, and non-invasive locoregional treatment approach in this setting. Case Report: A 61-year-old woman presented with unresectable breast cancer recurrence to the skin and subcutaneous tissue for which numerous lines of treatment were unsuccessful. Between February 2015 and May 2015, the patient underwent 3 courses of ECT after which a spectacular regression of the cutaneous metastatic foci was observed. After an overall observation period of 12 weeks, complete clinical remission was achieved. Conclusion: ECT can be proposed as an effective and safe locoregional therapy for breast cancer CWR and provides an alternative treatment modality to conventional therapies, especially in the case of multiple cutaneous and subcutaneous lesions.
