Drug reaction with eosinophilia and systemic symptoms (DRESS) is an idiosyncratic severe cutaneous adverse reaction (SCAR) characterized by a skin rash with systemic involvement (e.g., hematological, solid organ abnormalities). Various medications, most commonly anticonvulsants (carbamazepine, phenytoin), antibiotics (vancomycin, amoxicillin), and sulfa drugs (dapsone, sulfasalazine), have been implicated. We report a case of a 75-year-old man with pulmonary tuberculosis under anti-tubercular treatment (ATT Category 1 as per the national guidelines of Nepal) presenting with rash, fever, liver dysfunction, and eosinophilia, a combination of features suggestive of DRESS. According to the national tuberculosis (TB) survey of 2018-2019, over 117,000 people in Nepal were living with TB, including 69,000 newly diagnosed people. In third-world countries, such as Nepal, with a high TB prevalence, and the Southeast Asian region (with a huge percentage of the global burden of ‎TB‎ incidence), the risk of life-threatening adverse drug reactions during ATT is high. However, a good response is seen if it is recognized early and on stopping ATT and receiving a course of steroids and emollients.

Introdução: Lúpus eritematoso cutâneo crônico (LECC) é uma doença inflamatória autoimune, sendo características as lesões em pele. As manifestações intraorais são descritas e podem apresentar aspecto semelhante a outras lesões de origem autoimune, destacando a necessidade de avaliação e diagnóstico das lesões em mucosa oral pelo cirurgião-dentista. Objetivo: Apresentar um caso de lúpus eritematoso cutâneo com múltiplas manifestações orais, sem lesões em pele simultâneas, com enfoque em exames clínicos, histopatológicos e sorológicos para investigação de envolvimento sistêmico. Relato de Caso: Mulher de 54 anos, com histórico de lúpus, compareceu a clínica com uma lesão de aspecto liquenóide em lábio inferior e no palato duro observou-se uma área extensa, eritematosa e assintomática, com linhas brancas reticulares. Gengivite descamativa foi observada em maxila e mandíbula. Em mucosa jugal havia áreas liquenóides e pigmentadas. Biópsia incisional foi realizada no palato duro e a análise histopatológica revelou fragmento da mucosa bucal exibindo hiperqueratose e degeneração focal da camada basal. Foram solicitados exames complementares os quais descartaram envolvimento sistêmico. Conclusão: Esse caso destaca a diversidade de manifestações orais em paciente com lúpus e a importância do exame de mucosa oral, mesmo na ausência de lesões em pele, que são mais comuns. É fundamental a participação do cirurgião-dentista na esquipe multidisciplinar dos pacientes em tratamento de lúpus eritematoso.

Objective Discoid lupus erythematosus (DLE) is the most common category of chronic cutaneous lupus erythematosus, where the pathological process is proved to be closely associated with immunity. This bioinformatic analysis sought to identify key biomarkers and to perform immune infiltration analysis in the skin biopsy samples of DLE. Methods GSE120809, GSE100093, GSE72535, GSE81071 were used as the data source of gene expression profiles, altogether containing 79 DLE samples and 47 normal controls (NC). Limma package was applied to identify differentially expressed genes (DEGs) and additional Gene Ontology (GO) together with The Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were done. Protein-protein interaction network (PPI) was constructed using STRING and Cytoscape. Hub genes were selected by CytoHubba. Finally, immune filtration analysis was finished by the CIBERSORT algorithm, and comparisons between the two groups were accomplished. Results A total of 391 DEGs were identified, which were composed of 57 up-regulated genes and 334 down-regulated genes. GO and KEGG enrichment analyses revealed that DEGs were closely related with different steps in the immune response. Top 10 hub genes included GBP2, HLA-F, IFIT2, RSAD2, ISG15, IFIT1, IFIT3, MX1, XAF1 and IFI6. Immune filtration analysis from CIBERSORT had found that compared with NC, DLE samples had higher percentages of CD8+ T cells, T cells CD4 memory activated, T cells gamma delta, macrophages M1 and lower percentages of T cells regulatory, macrophages M2, dendritic cells resting, mast cells resting, mast cells activated. Conclusion This bioinformatic study selected key biomarkers from the contrast between DLE and NC skin samples and is the first research to analyze immune cell filtration in DLE.
