J Strength Cond Res 32(1): 230-236, 2018-The study was aimed at determining the acute effects of 2 types of occlusal splints on maximum isometric strength and fatigue of the elbow flexors muscles. The hypothesis was that splint-induced masticatory muscle repositioning might improve primary muscles recruitment by stretching masticatory muscles especially with the thicker splint. On 9 physically active volunteers with no temporomandibular joint and masticatory muscles disorders, we assessed maximum voluntary contraction (MVC) of the elbow flexors with diurnal (OSD, 1-mm thick) and sport (OSSP, 3-mm thick) splints, and without splint (control, Ctrl). On different days, participants performed 60 seconds of isometric contraction at 100% MVC (100%60s) and 80% MVC contraction until exhaustion (80%exh) under OSD, OSSP, and Ctrl in random order. Time of force output within target (t-target), force distance from target (ΔF), and force coefficient of variation were calculated. Percentage of force decay (ΔFi-Fe) was determined during 100%60s. From the electromyographic (EMG) signal, root mean square (EMG RMS) and mean frequency (EMG MF) were determined. Neuromuscular efficiency (NE) was calculated as the ratio between force and EMG RMS. MVC contraction and NE were significantly higher in OSSP and OSD than in Ctrl. During MVC, EMG MF was significantly lower in both splint conditions, and EMG RMS showed a nonstatistical tendency to lower values under both splint conditions. During 80%exh, t-target was longer in OSD and OSSP (+7.8% and +5.2%, respectively) than in Ctrl. ΔFi-Fe was lower in OSSP than in Ctrl and OSD. These results support the hypothesis of a NE improvement of the elbow flexors possibly induced by acute, splint-induced masticatory muscles repositioning.

Background:With continuous debate and lack of consensus regarding which are more eff ective in management of the temporomandibular disorders (TMDs), computer-aided design/computer-assisted manufacture (CAD/CAM) or conventional occlusal splint. Aim: The aim of this systematic review and meta-analysis was to perform a qualitative and quantitative analysis of the scientifi c literature regarding the use of CAD/CAM versus conventional occlusal splint in the management of patient with TMDs. Methodology: Manual and electronic database (PubMed and Cochrane) searches were performed to identify randomized controlled trials (RCTs), without language restriction, comparing CAD/CAM versus conventional occlusal splints. Data were extracted independently by two reviewers. The Cochrane tool was used for assessing the quality of included studies. Meta-analyses were performed for the two included RCTs of similar comparisons reported the same outcome measures. Results: Eight trials were identifi ed. Out of them two RCTs were included and six trials were excluded. The two included RCTs were considered to be at a low risk of bias. The pooled result revealed that the CAD/CAM occlusal splint has statistically signifi cant less pain in jaw joint, face tension, and time needed for adjustment when compared to conventional occlusal splints. Conclusion:The meta-analysis revealed that the CAD/CAM occlusal splint has statistically potential in reduction of pain in jaw joint, face tension, and time needed for adjustment when compared to conventional occlusal splints. Clinical Signifi cance: CAD/CAM occlusal splint may be suggested as an alternative manufacturing modality for TMDs patients who can off er the cost of this treatment modality. This review should be interpreted with caution because of limited number of included studies. Well-designed RCTs should be conducted and the outcome measures evaluation should be standardized between studies to reach potent conclusion.

Temporomandibular joint disorder (TMJ) is a complex and multifactorial functional disorder. Best approach in the treatment of TMJ disorders needs in detail proper diagnostic study. Joint vibration analysis (JVA) device, a new age technology and one of the most important diagnostic tools, is used for detecting intra-articular sound vibrations. Every type of vibration in different frequencies shows us the status of joint. Evaluation can be made after analyzing the results applying to a diagram. Like Combining of the sound vibration diagnostic techniques with other examination methods may be very meaningful in efforts of treating TMJ problems. Another diagnosis method is the evaluation of chewing movements. Best chewing efficiency is the most important purpose of masticatory system. Final product is a very important indicator for the efficiency of the mastication, and chewing pattern. T-Scan digital occlusal analyzing system is another important occlusal diagnostic instrument. Digital occlusal analysis system is currently the most powerful method of TMD clinics for treatment of patients with muscle pain dysfunction syndrome. Digital occlusal analysis system allows us to perform the MPDS treatments, splint and occlusal rehabilitation. The three important diagnostic systems are described in this chapter.
