Custom-made foot orthoses for the treatment of foot pain

Abstract: There is limited evidence on which to base clinical decisions regarding the prescription of custom-made foot orthoses for the treatment of foot pain. Currently, there is gold level evidence for painful pes cavus and silver level evidence for foot pain in JIA, rheumatoid arthritis, plantar fasciitis and hallux valgus.

“…The authors concluded that foot orthoses decrease pain and improve function in the short, medium and longer term (<6 weeks, 6 to 12 weeks and >12 weeks, respectively). The finding that foot orthoses are effective at all three time points is inconsistent with the finding of the review by Hawke and colleagues,22 and differs to the results obtained in this review. However, Lee and coworkers23 pooled each study’s orthosis group data for the respective time point and then compared the percentage improvement between the three time points.…”

“…The first meta-analysis by Hawke and colleagues22 reviewed the effectiveness of customised foot orthoses on all types of foot pain and included five randomised or controlled clinical trials that investigated PHP. The authors concluded that there was insufficient evidence that customised foot orthoses reduce pain compared with sham or non-customised foot orthoses at 12 weeks, or reduce pain at 6 to 8 weeks when used with a stretching programme or night splints.…”

“…Custom made functional foot orthoses needed to be a pair of in-shoe devices that were moulded or milled from impressions of the feet, whether by plaster cast or 3D laser scan. The fabrication then needed to be carried out according to practitioner-prescribed specifications 28. The CSI treatment had to involve an injection of a corticosteroid solution into the area of the plantar fascia.…”

“…The orthoses will be covered in a 2 mm blown multi-density EVA cover (Multiform) which is anticipated to provide shock absorption. There is currently no literature on the effectiveness of any custom, semi-custom or prefabricated orthotic device in the treatment of posterior heel pain associated with calcaneal apophysitis [35]. …”
