Custo-efetividade de cardiodesfibriladores implantáveis no Brasil nos setores público e privado

Abstract: Background: Many randomized clinical trials have demonstrated the effectiveness of the implantable cardioverterdefibrillator (ICDs) in death reduction of chronic heart failure (CHF) patients. Some developed countries studies have evaluated its cost-effectiveness, but these data are not applicable to Brazil.

“…Advanced treatments such as implantable cardioverter defibrillators or cardiac synchronisation therapy are potentially cost-effective in some places (eg, Brazil), but are expensive. 98 The use of thrombolytic therapy to treat ischaemic stroke requires timely access to CT; and, if built only to diagnose stroke, these facilities are as yet unaffordable. As costs drop, and skilled providers are trained, additional capacity to diagnose and manage more complex disease should become available in specialised facilities (panel 2).…”

“…Ribeiro et al42 evaluated the cost-effectiveness of Implantable Cardioverter Defibrillators in congestive heart failure patients under two perspectives in Brazil: public and supplementary health systems. They conclude that the incremental cost-effectiveness ratio of Implantable Cardioverter Defibrillators is elevated in the general heart failure population, in either the public or private perspective.…”

“…According to the guidelines for the economic evaluation proposed by the Ministry of Health (MS), the hospitalization costs obtained from the AIH values have been used in almost all studies conducted in Brazil, although the AIH is criticized for representing the values reimbursed and not necessarily the actual costs 6 . In this context, we can observe the use of the average value of the AIH, which was obtained from the analysis of the SIH/SUS, in studies with the most varied objectives, such as: disease cost assessment 2 , 8 , analysis of vaccine efficacy 9 , 10 , efficiency of new diagnostic technologies 4 , efficiency of programs 11 , and efficiency of new therapeutic technologies 12 , among others.…”
