“…Traditional posterior spinal arthrodesis with or without instrumentation in this age group should be supplemented by anterior arthrodesis to prevent crankshaft phenomenon. 21,22 Circumferential arthrode- sis will halt curve progression; however, at the same time, it prevents future spinal and thoracic growth. Furthermore, studies have shown that fusion surgery at an early age does not improve respiratory function.…”

“…This is determined based on the patient's Risser stage (stages 0-2), status of triradial cartilage (open), and amount of predicted residual curve (> 30°) after posterior correction. In a retrospective review of 86 immature patients undergoing posterior thoracic fusion for idiopathic scoliosis, Roberto, et al, [42] found that 75% of patients who were Tanner Stage 1 with open triradial cartilages developed the crankshaft effect (progression of > 10°) compared with no patients who were Tanner Stage 4 (unpublished data).…”

“…Postoperatively her curve measured 37° and the lateral subluxation of L-3 on L-4 was decreased to 4 mm (C, D). Note the use of interbody cages packed with rib autograft to [42,45] (Fig. 6).…”
