PurposeTo observe the efficiency of reirradiation with high dose rate intraluminal brachytherapy in symptomatic palliation of recurrent endobronchial tumors.Materials and MethodsBetween January 1994 and June 1998, 21 patients diagnosed with recurrent endobronchial tumors following external beam radiotherapy were treated palliatively with high dose rate intraluminal irradiation at Hacettepe University Oncology Institute. A single fraction of 10 Gy was prescribed to the specified area in 9 patients and 15 Gy to 12.ResultsEndobronchial treatment improved the performance and reduced symptomatology in 17 (81%) patients. Ten dyspneic patients (10/14, 71%) recovered clinically with an accompanying radiological downstaging. The median symptomatic palliation was 45 days (range, 0 - 9 months), and the overall median survival was 5.5 months (range, 4 - 12 months). The palliative intrabronchial brachytherapy was well tolerated, with the exception of in one patient with a fatal hemorrhage, and another with medically salvaged bronchospasm and intrabronchial edema.ConclusionRecurrent patients with a history of previous thoracic external beam irradiation can be effectively palliated with high dose rate endobronchial reirradiation if the symptoms are directly related to the endobronchial tumor.

Animal experiments were performed with evaluation of the use of the neodymium-YAG (yttrium-aluminum-garnet) laser in the trachea, bronchi, and pulmonary parenchyma. Histologic evaluation of incisions into the lung was assessed by multiple sections over a 5-week period and comparisons were made of incisions closed with chronic catgut or coagulated with electric cautery or by the laser. These studies indicated that the laser is more effective in controlling parenchymal bleeding and air leaks than either the cautery or chromic catgut and produces less local tissue reaction. The Nd-YAG laser has been used clinically to treat patients with inoperable obstructing and bleeding carcinomas of the trachea and main bronchi to open the involved airway and to restore pulmonary function. Fifteen patients have had 26 treatments using the Nd-YAG laser. Total laser time was limited to 15 minutes with 0.5-second pulses of 50 to 80 watts being delivered via the fiberoptic and straight bronchoscope as indicated. It is extremely effective in controlling hemoptysis from recurrent endobronchial lesions, and obstructing lesions in the trachea and main stem bronchus can be treated quite successfully with excellent palliation. It is apparent that continued clinical use of the Nd-YAG laser as palliative therapy for bleeding and obstructing tumors of the tracheobronchial tree is indicated and expanded use at the time of thoracotomy to control lung parenchymal bleeding and air leaks should be considered in the future.

A comparative study of laser soft tissue effects using carbon dioxide (CO2 ) and neodymiumyttrium aluminum garnet (Nd‐YAG) lasers in contact and noncontact endoscopic delivery systems was done in the canine tracheobronchial tree to determine the best laser system for the specific disease. The CO2 laser bronchoscopic coupler, CO2 laser waveguide, Nd‐YAG laser fiber, and Nd‐YAG laser contact probe were used. A 5 mm diameter soft tissue injury exposing tracheal cartilage was made, and the power density for each laser modality was measured. Soft tissue effects were documented histologically using routine techniques and scanning electron microscopy acutely and at 1 week and 2 weeks after injury. The effects were similar with the CO2 laser coupler and the CO2 laser waveguide with comparable power densities. Compared with the CO2 laser, the Nd‐YAG laser fiber caused more extensive and more difficult to control thermal damage in tissue. The Nd‐YAG laser contact probe was capable of precise soft tissue ablation with minimal surrounding tissue damage and was associated with the most rapid healing. This study concluded that high power density with short exposures is desirable when treating benign disorders, such as tracheal stenosis, making the CO2 laser or Nd‐YAG laser contact probe application a good choice. On the other hand, malignant turmores with a potential for hemorrhage are best treated with the Nd‐YAG laser noncontact fiber system because of its superior coagulative effects.
