Current Understanding and Clinical Management of Meniere's Disease: A Systematic Review

Abstract: Meniere's disease (MD) is a set of uncommon disorders with core phenotype of tinnitus, episodic vertigo, and sensorineural hearing loss. MD shows a genetic predisposition and a family history is found in 10% cases, with an autosomal dominant inheritance pattern. It is a multifactorial condition whose onset and development are triggered by the combined effect of genetic and environmental factors. Histopathological studies have associated MD with the accumulation of endolymph in the cochlea and the vestibular or… Show more

“…The main risk factor of tinnitus is hearing loss, but it is often associated with other conditions including hyperacusis, anxiety, depression, hypertension, insomnia or migraine [3]. Meniere disease (MD) is a rare inner ear disorder with a significant genetic contribution [4], characterized by episodes of vertigo, tinnitus and sensorineural hearing loss [5]. Although vertigo attacks are considered as the main symptom in the first years of the disease, persistent tinnitus is described as the most troublesome symptom by many MD patients [6,7].…”

“…However, short-term fluctuations of audiovestibular symptoms do not correlate with gross endolymphatic hydrops changes, suggesting that the fluctuations in hydrops associated with attacks have an amplitude below the limits of current MR resolution. 1…”

“…3 The etiology remains enigmatic but may involve heredity, auto-immunity, and inflammation. 4 The reason why some membranes in Meniere disease only distend while others rupture remains enigmatic. A doublehit mechanism that involves a change in membrane distensibility is theorized to play a role in this process.…”
