Diagnosis and Treatment of Bladder Endometriosis: State of the Art

Background: The bladder is the most common affected site in urinary tract endometriosis, being diagnosed during gynecologic follow-up. The surgical urological treatment might lead to good results. Study Objective: To define the state of the art in the diagnosis and treatment of bladder endometriosis. Methods: We performed a literature review by searching the MEDLINE database for articles published between 1996 and 2011, limiting the searches to the words: urinary tract endometriosis, bladderendometriosis, symptoms, diagnosis and treatment.Results: Deep pelvic endometriosis usually involves the urinary system, with the bladder being affected in 85% of cases. The diagnosis has to be considered as a step-by-step procedure. Currently, the treatment is usually surgical, consisting of either transurethral resection or partial cystectomy, and eventually associated with hormonal therapy. The hormonal therapy alone counteracts only the stimulus of endometriotic tissue proliferation, with no effects on the scarring caused by this tissue. The overall recurrence rate is about 30% for combined therapies and about 35% for the hormonal treatment alone. Conclusions: The bladder is the most common affected site in urinary tract endometriosis. Most of the time, this condition is diagnosed because of the complaint of urinary symptoms during gynecologic follow-up procedures for a deep pelvic endometriosis: a close collaboration between the gynecologist and the urologist is advisable, especially in highly specialized centers. The surgical urological treatment might lead to good results in terms of patients’ compliance and prognosis.

BACKGROUND: Glutathione-S-tranferase (GST) is the part of the key phase II detoxifying enzyme system. Many studies have investigated the role of GSTM1 and GSTT1 gene polymorphisms in endometriosis. Although GSTP1 was found to be one of the most abundant types of GST in genital system, there are insufficient data about the importance of the role of GSTP1 gene polymorphism in endometriosis. METHODS: This case -control study involved 150 patients with endometriosis and 150 controls. The frequency of GSTP1 single nucleotide polymorphisms was evaluated using PCR and melting curve analysis. RESULTS: The proportion of GSTP1 ile/ile tended to be higher in patients with endometriosis than control group, although the difference was not significant [odds ratio (OR) 5 1.53; 95% confidence interval (CI) 5 0.95-2.46]. In contrast, GSTP1 val/val was significantly higher in control patients and seems protective for endometriosis (OR 5 0.10; 95% CI 5 0.02-0.42). CONCLUSION: The results of this study suggest that GSTP1 polymorphism might modulate the risk of endometriosis with significantly decreased risk for GSTP1 val/val and marginally increased risk for GSTP1 ile/ile. Further studies on not only the disease processes but also normal distribution of the enzyme in female genital tract may provide better understanding about the role of GST types and their polymorphs in endometriosis.

Background: Recently, the role of oxidative stress in progression of endometriosis has been reported. Ischemia-modified albumin (IMA) is a marker of protein oxidation and very limited number of studies has evaluated the role of IMA in endometriosis. This study was designed to evaluate the serum and peritoneal fluid IMA levels in moderate/severe endometriosis as a marker for oxidative stress.Methods: This study was designed as a prospective controlled clinical trial. The study group consisted of 35 cases who underwent laparoscopy and with a diagnosis of moderate/severe endometriosis. The control group (n=35) was cases without endometriosis that underwent laparoscopy for tubal sterilization. The serum and peritoneal fluid IMA levels were measured spectrophotometrically by colorimetric method with complex of albumin non-binding cobalt and dithioerthreitol.Results: Although the median serum IMA levels in study and control groups were similar (p=0.553), the levels of peritoneal fluid IMA were significantly higher in study group (p=0.044). In endometriosis cases with dysmenorrhea peritoneal fluid IMA levels were much higher than cases without dysmenorrhea (p=0.018).Conclusions: The increased levels of IMA in peritoneal fluid of endometriosis support the possible role of oxidative stress in endometriosis. With this study, peritoneal fluid IMA levels are initially documented in endometriosis cases.
