Current status of second-look arthroscopy after meniscal allograft transplantation: review of the literature

Abstract: Although second-look arthroscopy has inevitably ethical issues, especially for asymptomatic and well-functioning knees, in this review, it could be a more dependable method available to detect meniscal healing if performed for certain indications such as a symptomatic patient with a magnetic resonance image indicating abnormalities.

“…Oh et al 101 found that second-look arthroscopy was performed after MAT in 100% of cases in 2 studies and about 46% of cases in 12 studies; the rate was not reported in 1 study. The authors were unable to find distinct indications for performing second-look arthroscopy, however most studies performed second-look arthroscopy for objective evaluation or to treat another knee problem.…”

“…Since the midbody and parts of the anterior and posterior horns of meniscal allografts are fixed to the remaining rim of the native meniscus/joint capsule, a vascularized meniscocapsular junction can be observed in well-healed meniscal allografts during second-look arthroscopy. 26 A circumferentially oriented disruption of the meniscocapsular junction and early meniscocapsular fixation failure were defined as meniscocapsular separation tears.…”

“…Beyond the patient's native anatomy, surgeons must consider the advantages and disadvantages inherent to the procedure ( Table 2 ). In the short to intermediate term, MAT has been shown to improve pain according to Lysholm and IKDC subjective scales 12 , 14 , 15 and regain lost function. 8 Long-term, it may be chondroprotective based on biomechanical evidence, 15 which ultimately may delay arthroplasty.…”
