Current state of melanoma diagnosis and treatment

Abstract: Melanoma is the deadliest form of skin cancer. In the early stages, melanoma can be treated successfully with surgery alone and survival rates are high, but after metastasis survival rates drop significantly. Therefore, early and correct diagnosis is key for ensuring patients have the best possible prognosis. Melanoma misdiagnosis accounts for more pathology and dermatology malpractice claims than any cancer other than breast cancer, as an early misdiagnosis can significantly reduce a patient’s chances of surv… Show more

“…The most important prognostic factor of melanoma is the BRAF mutational status [6]. The other prognostic factors are American Joint Committee on Cancer (AJCC) melanoma TNM (tumor, node, metastasis) staging [7], Clark level, and Breslow thickness [8], and they are useful for the clinical management of patients with melanoma. In the United States, patients present melanoma at different stages, with 84% of them presenting localized disease, 9% presenting regional disease, and 4% exhibiting distant metastasis [9].…”

“…PFS and MSS were calculated using Kaplan–Meier estimates, and differences between survival curves were analyzed by log-rank test 25 . The impact of age at diagnosis, gender, tumor location, Breslow thickness 37 , Clark level, TNM stage and genotypes of each analyzed SNV in survival of patients were evaluated using univariate Cox proportional hazards ratio (HR) regression. At a second time, all variables with p < 0.20 were included in the multivariate Cox regression.…”

“…In addition to external causes such as ultraviolet radiation, familial genetic factors cannot be ignored. Patients with melanoma diagnosed early and treated surgically had a higher survival rate; however, those with advanced metastatic melanoma had lower survival rates [1] . In recent years, the treatment of melanoma has included conventional chemotherapy and radiation therapy, as well as therapies targeting B-Raf proto-oncogene, serine/threonine kinase (BRAF) and MAP kinase-ERK kinase (MEK).…”
