Current salvage methods for recurrent prostate cancer after failure of primary radiotherapy

Abstract: We reviewed the current salvage methods for patients with local recurrent prostate cancer after primary radiotherapy (RT), using a search of relevant Medline/PubMed articles published from 1982 to 2008, with the following search terms: ‘radiorecurrent prostate cancer, local salvage treatment, salvage radical prostatectomy (RP), salvage cryoablation, salvage brachytherapy, salvage high‐intensity focused ultrasound (HIFU)’, and permutations of the above. Only articles written in English were included. The object… Show more

“…In this report, probabilities of biochemical-free survival, metastasis-free survival, and cancer-specific survival were 37, 77, and 83%, respectively [18]. Cryosurgery is another treatment option in patients with local recurrence after radiotherapy [7,16,19,20,21,22]. Although several definitions have been used, the disease-free survival rate was 40-58% at 5 years.…”

“…Representative options after postradiation biochemical recurrences include radical prostatectomy, cryosurgery, ADT, observation and brachytherapy (LDR or HDR) [7,16]. ADT has historically been favored for these patients because the treatment is simple to administer and in most cases can effectively control the disease for years [4,5].…”

“…Recently, a 5-year biochemical disease-free survival (bDFS) exceeding 50% was reported, according to the analysis of the Cryo On-Line Data (COLD) registry [4] , while a study with long follow-up demonstrated a 7-year bDFS of 59% [5] . Major technological improvements in the third-generation cryotherapy systems have also led to a more favourable safety profile [6] .…”
