Current practice patterns in nodal evaluation and adjuvant treatment of advanced stage endometrioid endometrial cancer: An SGO survey

Abstract: Highlights The majority of respondents evaluate lymph nodes via sentinel lymph node mapping for grade 1–2 endometrial cancer. Only 50% of respondents perform intraoperative sentinel lymph node mapping for grade 3 endometrial cancer. 90% of respondents give chemotherapy-based adjuvant treatment for advanced endometrial cancer. 75% of respondents combine radiation therapy with chemotherapy in stage III endometrial cancer.

“…We built on previously published surveys and then used extensive formative research to ensure inclusion of gynecologic oncology specific subject matter. Although our survey response rate was similar to other emailed surveys of SGO members ( Parker et al, 2020 ), there may be concerns about generalizability. Female and early-career members may have been over-represented compared to the general SGO membership ( Goff and Kushner, 2020 ).…”

“…The third limitation was that, considering the outcomes of recent clinical trials, we did not pursue some important factors, including the location of the tumor within the uterus or the type and extent of LN dissection (sentinel LN sampling vs. LN dissection), which may be important factors determining the need for EBRT. Performing LN dissection is another important issue to be investigated [ 18 ].…”

“…The use of adjuvant treatments remains somewhat controversial, partly due to low recurrence rates after surgery alone, although upfront surgery has remained the mainstay of treatment. Pathological evaluation is an important basis for oncologists to choose postoperative adjuvant therapy, especially pathological lymph node evaluation [ 15 ]. Our model can not only effectively stratify the risk of LNM, but also predict a high risk of recurrence in patients without LNM.…”
