Comparison of resin-based and glass ionomer sealants with regard to fluoride-release and anti-demineralization efficacy on adjacent unsealed enamel

This study compared resin-based and glass ionomer sealants with regard to their fluoride-release behavior and anti-demineralization potential on adjacent unsealed enamel surfaces. Sealant cavities prepared on bovine enamel blocks were filled with fluoride-containing resin sealants [TeethmateF-1 (TF), Clinpro TM (CP)], and glass ionomer sealant [Fuji VII (FVII)]. Specimens were then incubated in artificial saliva for 14 days to measure fluoride. Thereafter, demineralization was performed for 10 days, and the anti-demineralization efficacy was assessed by Swept Source Optical Coherence Tomography (SS-OCT), and cross-sectional nanohardness. All data were statistically analyzed by using ANOVA. FVII exhibited the highest fluoride release. SS-OCT and nanohardness findings indicated that anti-demineralization efficacy of TF was the greatest, whereas FVII was not significantly different from that of CP. Resin sealants released a lower amount of fluoride but exhibited anti-demineralization effects on the adjacent unsealed enamel surfaces that were comparable to that of a glass ionomer sealant.

“…Aproximadamente 90% das lesões cariosas em dentes posteriores iniciamse nestas regiões, seguindo as direções dos prismas de esmalte, e se apresentando internamente em forma de cone, com ápice voltado pra a região oclusal 2 . O correto diagnóstico dessas lesões é essencial para que se estabeleça o tratamento com melhor prognóstico em longo prazo, sendo necessário diferenciar as lesões que precisam de intervenção restauradora, daquelas que podem ser preservadas por meio de métodos não-invasivos 2 , incluindo o uso de fluoretos tópicos, dentifrícios, aplicação de selantes, melhorias na dieta, educação em saúde bucal e assistência odontológica, de preferência preventiva 16 . As indicações para tratamento restaurador em uma área de cicatrícula e fissura relaciona-se com a probabilidade de haver lesão de cárie significante na dentina.Geralmente lesões cariosas não cavitadas, restritas a esmalte e terço superficial da dentina e sem a presença de sintomatologia dolorosa,nãonecessitam de intervenção operatória restauradora, a menos que a lesãoesteja progredindo para porções mais internas da dentina 17 .…”

RESUMOObjetivo:O presente trabalho se propõe a discutir, asformas de tratamento para cada tipo de lesão cariosa após seu correto diagnóstico. Material e Métodos: foi realizada uma busca de estudos relevantes publicados nas bases de dados PUBMED, LILACS, SCIELO e MEDLINEaté junho de 2014.Resultados:Os sulcos e fissuras oclusais são regiões de elevado risco para o desenvolvimento de lesões cariosas, por ser uma superfície bastante retentiva para o acúmulo de biofilme. Essas lesões podem ocorrer também nas superfícies interproximais e nas superfícies lisas, sendo a primeira localizada rotineiramente mais na cervical abaixo do ponto de contato, e a segunda na superfícievestibular ou lingual/palatina. Para o tratamento das lesões incipientes,os fluoretos, como os vernizes e géis fluoretados, vêm sendo utilizados como instrumentos eficazes e seguros na prevenção e no controle da cárie dentária. Desse modo, a fluorterapiaconsiste em um importante elemento dos sistemas de prevenção à cárie dentária. Conclusão: O profissional deve adotar uma correta conduta terapêutica de modo a intervir com o mínimo de desgaste dentário e que seja efetiva no controle e paralisação da doença. Na dúvida quanto à conduta a ser empregada a melhor opção é empregar medidas de promoção de saúde, esperar monitorando o caso e reavaliar posteriormente o quadro; visto que desgastes na estrutura dental devem ser feitos apenas quando for de extrema necessidade. ABSTRACTObjective:This literature review discussesthe treatment modalities for each type of carious lesion after their correct diagnosis. Results:Occlusal pits and fissures are high-risk areas for the development of carious lesions due to theirvery retentive surface, which favors biofilm buildup. These lesionscan also affect interproximal and smooth surfaces of the tooth, the first located in the cervical regionbelow the contact point, and the second surroundingthe buccal and lingual/palatal surfaces.Fluoride, such as fluoride varnishes and gels, have been used effectively and safelyin theprevention and control ofincipient caries lesions. Thus, fluoride therapyconstitutesan important approach tothe prevention of tooth decay. Conclusion: the dental professional should adopt a correct therapeutic approach to intervene with minimal tooth wear and maximum effectiveness in the control of the disease. In case one is unsureof the most appropriate conduct to be followed in a given situation, it is advised to employ health promotion measures, follow-up and then reassess the case.Tooth wear should beperformed only when extremely needed. Abstract: Current possibilities in occlusal caries prevention and management are very effective. Therefore, dentists today do not have any excuse for avoiding the philosophy of Minimally Invasive Dentistry, especially when we talk about caries management of occlusal surfaces in permanent molars.
