рефератЦель исследования -оценить клинико-рентгенологические результаты эндопротезирования коленного сустава с использованием индивидуальных направителей для позиционирования резекционных блоков, созданных с при-менением физических прототипов и 3D-печати.Материал и методы. за период с 2015 по 2016 г. нами было выполнено 4 операции тотального эндопротези-рования коленного сустава по поводу посттравматического гонартроза III-IV стадии. Операции выполнялись с использованием индивидуальных направителей для позиционирования резекционных блоков. Средний воз-раст пациентов составил 59±12 лет. Изначально у всех пациентов отмечалось выраженное ограничение дви-жений в коленном суставе. Средний объем сгибания до операции составил 53,3±35,1°. по данным телерентге-нографии (FLFS), у всех пациентов имелась варусная деформация нижней конечности со средним значением 15,0±12,3°.Результаты. Во всех случаях после операции нормализовалась ось нижней конечности. Средний балл по шкале KOOS по признаку «боль» составил 85,2±14,0; «ежедневная физическая активность» -74,5±14,8; «сим-птомы и тугоподвижность» -69,0±11,0; «качество жизни» -62,5±12,5 баллов. Средний объем движений в ко-ленном суставе после эндопротезирования: сгибание 113,3±5,8°; дефицит разгибания отсутствовал. поскольку у всех пациентов отмечалось выраженное ограничение движений в коленном суставе до операции, результат мож-но рассматривать как отличный.Заключение. предоперационное планирование с использованием индивидуальных направителей и прототи-пирования на физических макетах большеберцовой и бедренной костей позволило добиться нормализации оси нижней конечности у всех пациентов. показаниями к применению персонифицированных прецизионных напра-вителей могут быть наличие в анамнезе воспалительного процесса или деформации бедренной кости; эндопро-тез тазобедренного сустава на ипсилатеральной стороне, когда желательно избежать вскрытия костномозгового канала; значительные костные дефекты или массивные остеофиты задних отделов мыщелков бедра; выраженное ограничение движений в коленном суставе.Ключевые слова: тотальное эндопротезирование коленного сустава, 3D-печать, индивидуальные прецизион-ные направители.

Improving MiniHip femoral prosthesis positioning using a cross‐laser projection system in total hip arthroplasty by an anterolateral supine approach

Background The authors developed a cross‐laser projection system (CLP) to place a femoral neck‐sparing short stem using the minimally invasive anterolateral supine approach in total hip arthroplasty. This study aimed to verify the utility of CLP. Methods Thirty joints were assessed with the MiniHip (Corin). The authors compared femoral component implantation with a patient‐specific femoral osteotomy guide (PSG) for the femoral neck‐cut (PSG group), with the CLP attached to the rasp handle to irradiate the cross‐laser to the target of PSG (CLP group), and without PSG or CLP (control group). Results In the CLP group, the positional deviation of anteversion, anterior/posterior tilt and varus/valgus placement of the stem postoperatively were 1.8° ± 0.2°, 2.0° ± 2.0° and 2.0° ± 0.1°, respectively. The positional deviation of anteversion (p < 0.001) and anterior/posterior tilt (p = 0.036) were significantly smaller than those in the other groups. Conclusions CLP improves the accuracy of MiniHip femoral prosthesis placement.
